[Question #12736] Unknowing of what I have Oral

Approximately 2 1/2 weeks ago, I was possibly exposed to an STD or an STI. With a lack of judgment, I participated in fellatio, no receiving, only giving to a  transsexual male (MTF) with ejaculation involved. After the encounter, I threw up the ejaculation from my stomach and used mouthwash and brushed my teeth. 

After about one week, I started noticing that my tonsils were red and it felt like something was in the back of my throat, but I couldn’t clear it, my throat was also showing signs of bumps such as cobblestone throat. I do not have a fever. 

I went to an urgent care to get swabbed in will receive results in a few days, but I am unsure of what this will be as this is very unusual for me as I do not get sick often. I tested negative for strep.

I have been monogamous with my partner for the past four years and have had no other exposures in been tested prior with all negative results. 

The doctor believes it’s oral gonorrhea, but is unsure at this time. This is my only encounter and my partner has had no other encounters. 

What are the likelihood or probability of contracting HIV, syphilis, chlamydia gonorrhea, HPV or a HSV. I have been worried nonstop about what this could be and I do not want to have my partner infected as she is unaware.

I am anxious to know what it is or what it could be.

Welcome to our Forum and thanks for your questions, as well as your implied confidence in our service.  I'll be glad to comment.  Overall, performing oral sex is a relatively low risk form of penetrative sexual encounter - lower than genital-genital or ano-genital sex.  Further most people do not have STIs and most single exposures to infected partners do not lead to infection.  With regard to your specific encounter:

The most common STI acquired from performance of oral sex is gonorrhea which is most typically asymptomatic.  Syphilis and chlamydia are uncommon.  If you were going to acquire HSV and experience an outbreak you would have done so within 10 days of your exposure.  The risk of acquiring HIV, IF your partner had untreated HIV is less than 1 in 10.000, i.e. like performing oral sex on an untreated infected partner daily for over 40 years.  HPV is likewise unlikely.

I think you are worrying more than you need to.  Your gonorrhea test results will be definitive presuming your throat swab was sent for gonorrhea testing.

Thank you so much for your quick response. I respect and understand your analysis. One of my follow up questions would be that I noticed that there is a particularly large red vein in the back of my throat, could semen have reached to the VA vein and gotten into my blood somehow, I understand it can be a silly question.

I will wait for my next follow up until I get the results of the swab.

This prior question was in reference to the possibility of HIV. 

When the action occurred the semen was quickly swalled, but about 15-20 minutes later thrown up and rinsed away with listerine.

The fact that you can see a large vein in the back of your throat in no way changes my assessment and does not represent increased risk for HIV in the unlikely situation that your partner had untreated infection.

I took an Oraquick Exam and got a negative result(yay). I received a call back from the Urgent Care facility regarding my test results for oral chlymidia and oral gonorrhea and they said my tests were inconclusive…. I am experiencing minor symptoms like a ticklish throat and two white spot/possible tonsil stones on the back of my throat. No more green phlegm anymore. Should I go ahead and retest?

I’m needing assurance of the next actions?

I have also noticed a red/irritated spot this morning under my tounge behind my teeth on my gums… not sure if this is syphillis… I am paranoid about STI/STD’s. I have had canker sores in my mouth growing up as a child and do not want to misdiagnose this concern. I do apologize about the anxiety questions.

I'm not sure why your results were inconclusive .  Was there something wrong with the test?  If so, they should offer you a re-test at no charge.  The lesion under your tongue that you describe sounds nothing like syphilis.  

As I've already tried to indicate, the likelihood that you have an STI is quite low.  It is far, far more likely that you have coincidentally acquired one of the common community acquired, non-STI sore throats that most of us get from time to time.  From the sounds of things, you need the reassurance that a negative test would provide.  When you test, I'm confident your test will be negative.  This is true not only for gonorrhea and chlamydia but also for syphilis.  If this were syphilis, your syphilis blood test would be positive.  EWH

Good morning, I just received my test results for chlamydia and gonorrhea… Negative! Now that leaves the question of what are the possibilities of being HIV or syphillis from the encounter and when is the appropriate time to test for these? - id like to knock those out of the picture to put my mind at ease.

I am still experiencing some throat congestion (which I believe is being caused by my active GERD issue) and still swollen tonsils. I don’t believe I’ve had a fever as the days I’ve been ranging in between 97.8 - 99.1. I have not noticed any notable sores in my mouth or any rashes on my body.

I’m not surprised to hear the results of your test for gonorrhea and chlamydia. It saddens me to see that you continue to worry so much. If any of the symptoms you are concerned about are due to syphilis or HIV tests will be positive at this time. On the other hand, if you have either of these infections and your symptoms Are not due to those infections testing for HIV and syphilis will be entirely conclusive six weeks following your lower risk exposure.
