[Question #12763] FTM oral experience

I’m back. Unfortunately this time I have a bit of a change in situation. I met up with a FTM person who showed me all of their recent STD testing from 1/28/2025. All showed negative/non-reactive. They claimed to not of had any sexual experiences since those results. They performed oral on me for maybe 20 seconds and then I stopped them however what’s different is that I had a small blood blister on the shaft of my penis from a zipper injury that occurred some weeks back. I was messing with it a few hours before meeting this person and it popped. I cleaned the area and it stopped bleeding. I didn’t plan on meeting this person at that time but later that night I did and hadn’t thought about it until I was in the moment and that’s when I stopped the interaction. I of course have read as much as I could here and everything that the experts say is that mouth to penis is very low risk, no proven cases of hiv even in the event that the penis would have a small cut. However my paranoia and anxiety about the matter has me asking again

Welcome back to the forum.

I'm glad you understand the low STI risk from oral sex, in particular oral to penis. You don't say the precise timing of your recent oral sex exposure and how long it was prior to that event that "messed with" the penile injury site. But assuming you didn't have an obvious open wound or bleeding at the time, this probably made no difference in risk. In fact, even a fresh new wound would have made little difference. And your describe a partner who unlikely to have HIV or any other active, transmissible STI. Based on all this, I would not advise testing for anything; and if you have a regular partner, you can safely continue your usual sexual practices.

Perhaps I'll have more to say when I understand the timing. I hope these comments are helpful in the meantime.

Thank doc. Timing wise maybe 3-4 hours before the sexual experience the tiny blood blister had popped where I cleaned it up bleeding stopped there after

At the time of the sexual experience, I noticed no bleeding or an open wound during or after

OK, thanks. This doesn't change my assessment or advice. Nobody can say the STI/HIV risk is zero, but it remains exceedingly low. Considering how common oral sex is, and that penile sores etc aren't rare, there must have been millions of exposures (if not billions) in the present of such wounds -- and still no known cases of HIV from such events.---

I appreciate your response. I have one last follow question. I understand this has been your field of research since the 70s. You’ve also received awards in this field of study. Additionally between this form and your previous one you’ve answered tens of thousands of questions between you and Dr. Hook.

We know what the medical literature says CDC and so forth. But you have direct in the field experience so I’m curious to know out of all these tens of thousands of instances that you have reviewed. How rare is it that someone comes back and says actually I’ve been diagnosed with..

I can’t imagine that happens often

You're right in your closing statement, that such events are rare. To my best recollection, I've never been aware of any person who acquired HIV by oral-genital contact. In addition, in the 21 years of this and our preceding forum at MedHelp.org (which no longer is active), with thousands of questions from people concerned about HIV after a possible exposure, not one has yet reported that they tested positive. If and when it finally happens, I am confident it will be from a genuinely high risk exposure -- something like unprotected vaginal or anal sex with a known-infected partner, anal sex between two men, etc.
