[Question #12795] HIV exposure risk assessment from elite controller

I have two risks:
Risk 1: Getting a tattoo
Risk 2: Sexual intercourse with a female 28 days after Risk 1.
The female I had sexual intercourse with has done these tests: At 38 days: 3 rapid tests + 1 Ag/Ab machine test, all negative. At 42 days: Ag/Ab machine test negative. At 81 days: 3 methods (HIV Combo Ag/Ab Abbott, HIV DUO Roche, HIV Bioline 1/2 3.0) all negative and PCR RNA viral load undetectable. At 242 days: rapid Ab test at hospital negative.
For myself, I've done these tests: At 13 and 33 days after intercourse, rapid Alere combo test (while taking cold medication). At 35 days after Risk 2, I thought I had tuberculosis so I bought TB medication and methylprednisolone and took it for 1 week with no improvement, so I stopped. At 70 days after intercourse, due to continuing symptoms, I started taking TLD (ARV) on my own and haven't stopped yet. I had rapid tests using early detect (Alere combo) on days 90 and 108 (while still taking TLD during testing) with negative results. Ag/Ab machine test on day 164 after intercourse (while still taking TLD during testing) was negative. PCR RNA viral load test on day 165 after intercourse (after taking TLD continuously for 94 days until testing) showed undetectable viral load. Please evaluate both risks thoroughly, especially the possibility that the female could be an elite controller, so the tests performed would be false negatives. As for me, could it be that because I've been taking TLD since day 70 (still within window period), my body hasn't had enough time to produce sufficient antibodies to test positive with the tests performed, causing false negatives?

Welcome to our Forum. Thanks for your questions. I think you have seriously over reacted to the possibility of having HIV and possibly TB as well. You do not have HIV. I encourage you to see a trained health care provider and stop self-medicating yourself. If there is a problem, your actions are only delaying correct diagnosis and and management.
Getting a tattoo from a professional tattoo artist without shared needles is a no risk activity.
There is no way the female you describe could have given you HIV as her test results prove that she does not have HIV. She is NOT an elite controller. Even elite controllers have positive tests and her negative tests prove that she does not have HIV. Your own tests also prove that you do not have HIV. Taking TDL has been a waste of time. It is long since positive tests would have developed if you had been infected by either of the events you describe.
I'm not sure why you are so worried. You have proven you do not have HIV. As I said above, if you are ill I would encourage you to see a trained health care provider for evaluation. EWH

Thank you, Doctor. I have typical symptoms of HIV exposure with corresponding timelines. The symptoms include fever, swollen inguinal lymph nodes, a rash, and notably, an oral Candida infection confirmed to persist until I took antifungal medication for three months. All symptoms appeared after I had sexual intercourse with the female partner I described.
Doctor, could you please assess whether starting TLD (ARV) on day 70 would allow antibodies to develop fully? I understand antibodies typically take three months to reach detectable levels. Additionally, what further tests should I undergo to confirm my status?
I am aware the female partner is unlikely to have HIV, but I’ve read studies suggesting elite controllers might not test positive on standard antibody tests. Could this affect my diagnosis?

Your symptoms are due to something other than HIV.

Antibodies would have certainly developed by day 70, despite taking TLD. You do not need further testing. You have proven you don’t have HIV three ways - with tests for antibodies, for HIV antigen, and for the virus itself. Give up on the idea that you have HIV. Further testing is a waste of time and resources.
Your reading on elite controllers has misled you. Elite controllers have positive tests, including PCR.
