[Question #12869] Worried about HIV

Hello Doctor,
Recently a day ago I had an exposure with a CSW in india Mumbai she gave me an oral with condom and we had a vaginal intercourse with condom the whole thing from oral to vaginal intercourse was hardly 2-3 minutes. The room was all shady and dark and i am 80 percent sure that the condom did not break but again the room was all dark and also I also feel the csw was a transsexual. The next morning I texted the csw asking is she a trans she said she is straight and also asked her if she gets tested regularly she said yes and said she only offers sex with condom(she might be lying). Still I was worried what if the condom broke during the act and Out of anxiety I approached a local doctor she said the risk is very low but I insisted for pep and was given Dolutegravir Emtricitabine tenofovir 50mg/200mg/25mg one tablet every day for 28 days.
My questions are
1) I just had pep for one day should I stop taking it because I am having side effects like nausea and headache
2) if I had pep for one day and stopped it afterwards when can I be tested can I give a test after 28 days?
3). Should I be worried if the partner was a trans?
3) Am I overeacting to the exposure I had, cause I feel it’s a low risk exposure and my penis is circumcised but still I’m panicking about the whole HIV thing.

Welcome to our Forum. Thanks for your questions and your implied confidence in our service. I'll be glad to comment. FRY, no one has EVER been proven to acquire HIV from receiving oral sex from a n infected partner and even in the unlikely circumstance that your partner had HIV, your risk of acquiring HIV even if the condom broke would be less than, on average, one infection in over 2000 sex acts. while PEP is a personal choice, I would not have prescribed it for you in the situation you describe.
Further, you used a condom which you only imagine broke and your partner told you she did not have HIV.
In response to your questions:

The symptoms you describe could well be due to the medication you are taking or anxiety. I would not have prescribed it in the 1rst place. You decided to take PEP, you need to decide whether to continue or not.

No

Thanks for the reply doc
I will stop taking the pep from today this is my second day as I feel I am overreacting to the whole situation.
My followup questions are
1. Do you recommend me taking a test, if you do should I take the 4th generation hiv test after 28 days or should I give it more time to make sure the results are conclusive.
2) I forgot to mention that during the act we did have some naked body rubbing just for few seconds and she kissed on my chest and I did the same on her breast.

I agree that you are over reacting and think that stopping the PEP at this time is a good idea.
1. If you wish to test in follow up, at 28 days after your last dose of medication a 4th generation test will detect over 98% of recently acquired infections. Given the very low risk nature of your exposure, I would not feel the need for further testing after having a negative test result at day 28.
2. Thanks for the additional information. Body rubbing (frottage), kissing her breast and having her kiss you on your chest are no risk activities and do not change my assessment

Dear doc,
I am almost certain as I just enquired from her friend that the CSW is a trans and got her gender affirmation done few year ago (vaginoplasty) but I am sure the condom did not break or leak during the vaginal intercourse.the oral and intercourse we’re with condom only. Now that it is confirmed she is a trans who got her gender affirmation surgery done few years ago did my risk increase from that exposure?
