[Question #12902] STD possibility?

Hi Doctor,

I am a male and I had insertive vaginal sex and received oral sex from a female of unknown status. Before having sex I took PrEP on demand. I wore a condom and from what I saw, the condom didn't break. 2 weeks later I came down with body aches and I've been sneezing a lot. My body aches are very minor. I've been able to go to the gym and work out. I just feel kind of off. Could this be HIV ARS? 

I also went to a massage parlor a couple days ago and received a table shower where the masseuse bathed me and gave me a hand job. Is this a risk for HIV or an STD? I was naked, lying down (front and back) on the table shower and I'm afraid I could have come into contact with a previous individuals semen or blood. The table shower looks like this for reference

Welcome to the forum. Thank you for your question and for your confidence in our services.

You say nothing about your partner to judge the chance she might have HIV or any STD. It would also help to know where you are:  the risk might be greater in some geographic areas (like tropical Africa) than others. Simply being "of unknown status" doesn't help very much. However, assuming you are in North America or Western Europe, the large majority of sexually active women do not have HIV, and most do not have any other transmissible STD. In any case, receiving oral sex is close to zero risk for HIV and low risk for all STDs. PrEP usually is not advised except in much higher risk situations than this exposure seems to have been. But it also contributed to a virtually zero risk event in regard to HIV.

Your symptoms do not suggest a new HIV infection or any other STD; these do not cause sneezing. And contact with other persons' blood or body fluids in the environment never results in HIV or other STD. I see nothing in the image you posted that implies any risk.

In general, testing isn't recommended for either HIV or other STDs after any single exposure, especially given the protections involved (low risk for oral sex, condom for vaginal sex, or PrEP) and the low chance your partner is infected. However, testing is a personal choice, and reassurance is a valid reason for testing even when there is little or no actual risk. If you choose to be tested, I would advise only a urine gonorrhea and chlamydia test (valid any time more than 3-4 days after exposure) and blood tests for HIV and syphilis after 6 weeks. (Or an HIV RNA PCR blood test after 11 days, although this is quite expensive.) If somehow I were in your situation, I would not feel a need to be tested at all -- but it's your choice if you remain concerned.

I appreciate the quick reply. I live in the west coast of the United States. I'm glad to hear about the virtually zero risk exposures I've had. 

After the table shower exposure I started PEP (Truvada and Tivicay) just in case. I'll be discontinuing it since you say it was zero exposure. Strangely, my body ache symptoms went away a couple days after taking the Truvada and Tivicay. Is this proof that I was having HIV ARS from the exposure 2 weeks prior and the medications stopped it? Or is it just a coincidence? Sorry, it's just strange that it happened.

I’m glad to hear your symptoms cleared very quickly. It almost certainly means they were psychological, simply due to your worry about HIV. If due to HIV, the treatment could not have helped so quickly. ---

Hi Dr. Handsfield,

Apologies for the late reply. Thank you so much for the information! So just to summarize, these symptoms I experienced were not HIV ARS and I don't need to be concerned about HIV nor is there any medical reason for testing. Therefore, I can move on with my life and I don't need to worry about the possibility of infecting others. 

If that's correct, I'll trust you and move on. Thank you so much for your help.
