[Question #12919] STI Risk

I need to check. I had protected sex with a CSW. The condom seemed intact after. Would a small hole, even a pinprick risk infection (paranoid I know). No more than a few minutes. She also performed unprotected oral on me. I don’t want to risk sex with my partner whom I’m back with. I’ve never cheated, and will resume monogamy now we’re back together. What risks have I ran, even at the base of the penis? HIV, gonnorea, clymidia?

The bottom line is that this sounds like a pretty much risk free sexual event. As long as the condom appeared intact, it was. The notion of small leaks that can increase STI risk is pretty much an urban myth; if a condom doesn't break wide open, protection was complete. Even with no condom at all, the large majority of vaginal sex events with female sex workers do not result in any STI. It is normal for the base of the penis to be exposed directly, which is the main reason condoms are inherently less effective against those STIs transmitted skin to skin (HPV, herpes, syphilis) than those transmitted by sexual fluids (gonorrhea, chlamydia, HIV). But the risk still is very low for such infections.

Assuming you do not develop discharge from the penis or painful urination in the next several days, you can safely assume you don't have gonorrhea or chlamydia. If no penile sores or blisters within 2-3 weeks, syphilis and herpes will be very unlikely. In general, STI testing is not needed or recommended after any single exposure unless a lot higher risk than yours, and from a medical/risk standpoint I do not advise testing for anything. However, some people are more reassured by negative testing even when the risk is near zero; reassurance itself is a valid reason for testing. If you decide to test, I would advise a urine gonorrhea/chlamydia test 4-5 days after the event; and HIV and syphilis blood tests after 4 weeks for 95-98% assurance; or 6 weeks for 100%.

Personally, if somehow I were in your situation, I would be comfortable having unprotected sex with my wife after a week with no symptoms, and with no testing for anything. I hope these comments are helpful. Let me know if anything isn't clear.

Sorry I didn't comment on the oral sex risks. Very low for all STDs -- and this doesn't change my advice about optional testing.---
