[Question #12942] Worried/ shall i test again

My exposures : 3
16 march - protected vaginal
19 march - protected vaginal
22 march - protected vaginal/ unprotected oral

Location : thailand csw’s

I tested on 22nd april - 4th gen combo p24 agab eclia ( 30/31 days) after last exposure.

I also did a std panel ( blood urine) same day.

All tests came negative.

Doctors at labs ( not my family doctors) asked me to move on. 2 of them. All on phone. I also am taking a psychiatric session from same lab and counsellor today said they never saw 30 day test turn positive she will guide me to
Move on.

But i am a bag of nerves. I have abstained from my regular partner.

Shall I test more/ shall i move on?  Resume regular sex. 

Std test also included hepatitix b/c, hsv 1 and 2, chlymadia, syphillis, and 2 more . gonnohorea was not there. All negative 

Please help doctors.

Sorry to add : but i never noticed a condom break, but never actively check. So what are the chances i might have missed. Also i have lost 1.5 kg weight and having minor / very minor gases . Is it because of worry? I am also worrying 24*7

Welcome to the forum. Thank you for your confidence in our advice.

With only 3 exposures, only female partners, and consistent condom use, your risk of HIV was zero or close to it. And your negative AgAb test at 30-31 days is 98-99% reliable. Considering the low risk plus the test result, you can be 100% certain you do not have HIV. Therefore I agree with the laboratory doctors that you can move on without further worry. However, if you feel you need a negative test that is truly 100% conclusive, have another AgAb test 45 days or more after the last exposure. The other test results you report also are conclusive, although the on rare occasions it also can take up to 6 weeks for conclusive syphilis testing, so consider that test as well if you re-test for HIV after 45 days.

Your symptoms mean nothing in regard to HIV or any other STI; and although it can take 45 days for a positive HIV test, the results always are positive in presence of HIV symptoms. So your current result is proof HIV is not the cause of your gassiness and weight loss.

In summary, you definitely should not be "worrying 24/7" and you should be continuing unprotected sex with your regular partner. There is no chance you will risking infecting her with HIV or any STI.

I have a follow up.  

1. How likely is a 30 day test to turn positive at 45 days? Have you ever came accross this scenario?

2. I never noticed condom break.  But you never know.  That is my main concern.

3. The testing is very traumatic for me. My heart does not want to test again. I am sorry to put you on spot , but i am not my usual self - so  if you were me will you test again.

4. You also mentioned , i may test again - may - made me think its upto me , but medically or practically i am clear.

5 . I understood about syphillis, but what about hepatitis , any guidance there.

1. As I said above, the HIV AgAb test detects 98-99% of infections by 30 days. Given your low risk plus that result, the odds a later test will be positive are under one chance in millions.
2. It's hard to miss a condom break, but it doesn't matter anyway. Even without a condom, you would have been at little risk.
3. On one hand, you didn't need testing at all given the low risk of the event. But on the other, if you're going to keep worrying, you must be tested again. It's never appropriate to delay testing for fear of the result. When finally done, anxiety always is reduced even if the result is positive. The anxiety, fear and uncertainty are worse than just getting it done.
4. Correct -- "medically and practically" you were never at risk and now are in the clear.
5. Viral hepatitis is no longer an STD between males and females in most countries. Hep C has never been an STD between males and females, only between men having potentially traumatic anal sex; and hep B is as rare as HIV because most people are vaccinated.

Oral sex is no risk for HIV and very low risk for all other STIs.

Your closing statement is correct.

Hello Doctor, 

I tested for Hiv exactly on 45th day again and got a negative result. My questions and lingering worries below. I want to get out of this hole desperately. 

1. On my 31 days test 4th gen p24 ag ab test, value was 0.171 and now on 45th day it was 0.307. I probably know it doesnt matter, but need some assurance from you. 
I also hope i am clear from HIV.

2. I am worried of Hep B , The 30 day HSBAG ECLIA had 0.379 value. I am
Not sure another test is warranted considering this and my exposure mentioned above. But i am scared.

3. Hep C and Syphillis : i am
Probably not worried after my first tests and your responses and my exposure combined. I dont know why

4. I am still abstaining from regular sex. Depending on my therapy and your guidance, can i start with protected Atleast? Will that be safe.

Sorry for messaging sporadically, but i checked with clinic doctor again, after the 30 day test.and he adviced me for hepatitis b vaccine , but seemed dismissive, almost irritated at my concern.

It's harder to respond when there are multiple questions with information spread over several of them. Going to your numbered questions yesterday:

1. These are negative results for HIV that have no chance of later becoming positive. You do not have HIV.
2. Hepatitis B virus is little or no risk from the exposure described and the result is at least 95% proof you were not infected.
3. This question makes no sense:  you say you're not worried but don't know why (???)
4. There is no possibility you have HIV or any STI. You can safely resume sex with your regular partner.

Your negative hepatitis C test is conclusive, but hep C was no risk at all. Contrary to common belief, it is NOT sexually transmitted between males and females -- only between men who have traumatic (bloody) anal sex. 

That completes the two follow-ups included with each question and so ends this thread. I hope the discussion was helpful. If there is anything you don't understand, please do not start a new question. There is nothing you will think of that has any possibility of changing our evaluation and advice.
