Hello Doctor. Thank you for your wonderful service.
I have a question on my recent exposure(unprotected oral sex), i took another Hiv RNA PCR test at day 18 which is negative.
Could you please help me if this test is conclusive or i need to go for 4th Gen ag ab test at 4 weeks/6th week.
Questions-
1) What is the time line for viral load around 3 weeks time, is it enough to detect infection.
2) If 4th gen detects antigen between 3-4 week for 95% population(read it on viral test timeline) if this assumption correct, if yes then there should be enough viral load around 18 days to detect infection.
3) is there chance of this result still false negative?
4) is its true for some people viral load can only be detected after 4 weeks after exposure.
Thank u! I am sorry that i have to return to you with few more questions.

I don't understand why you're back. Dr. Hook's replies to your very recent question included "As I already told you, results of HIV RNA PCR tests are conclusive ANY time after 11 days. Your 12 day result is conclusive." What was unclear about that statement?
---
1) What is the time line for viral load around 3 weeks time, is it enough to detect infection. Yes.
2) If 4th gen....: Viral load and antigen are not the same thing, and it takes longer for antigen and antibody to be detected than RNA (viral load).
3) is there chance of this result still false negative? No.
4) is its true for some people viral load can only be detected after 4 weeks after exposure. No. In past years it sometimes took that long, but tests have improved since then.
"I am sorry that i have to return to you with few more questions." Especially because it was entirely unnecessary! As Dr. Hook said last time, "There should be no need to return with further questions. If you do, the question may be closed without a reply and without return of your posting fee." That is what will happen to any further repeated questions that already have been answered.

Hi Doctor Handsfield, thank you for your reply.
I have one more question: cdc and other websites says, window period for hiv rna qualitative test is 10-33 days. Why is window until 33 days to conclude 99%.
If my results at day 18 is conclusive.
Thank u!

First comment about RNA testing window period: See yesterday's reply no. 4; newer tests are conclusive sooner than older tests.
I'm sorry, but I do not "understand your thought process". I wonder if there is a basic misunderstanding perhaps due to a language barrier? I do not know how to make my replies any more clear or understandable.
You do not need another negative test result, but even when RNA testing is conclusive, some persons at risk like having the additional conclusive AgAb test result at 45 days. It's your choice.
Also, please note that nobody ever gets HIV from receiving oral sex; you were not at risk for HIV and did not need testing.
That concludes this thread. Please note the forum policy against repeated questions on the same topic. This being your second, in which you repeated the same questions as in your first thread, this one must be your last. Repeated questions may be deleted without reply and without refund of the posting fee. Thank you for your understanding. I do hope the two discussion have helped you. If you remain worried, I suggest you go back and carefully re-read both discussions.
