[Question #12990] HIV/STI oral sex risk & testing

Hi Drs/Team. I’m sorry to be back. Just seeking clarity (peace of mind) regarding my risks of HIV/STI from receipt of unprotected oral from a female. In Brief. I have had several full body massages some over the past few months many of a non-sexual nature and some with a receipt of a hand job only.However, on this occasion (today) I participated in a full body massage and received unprotected oral sex only. Based on advice within this forum I suspect that this is a no risk situation for HIV however given my new experience this has caused me some angst thus seeking clarity on my concerns. My queries are: 1. Have I put my health at risk for HIV other STIs from this event.2. Do I need to test for HIV/STIs from this event and if so how soon, for what and when to test to be conclusive. 3. Do I need abstain for a period of time from normal sexual practices with my regular partner. PS. No sexual penetration vaginal/anal ever and only with regular partner.  I’m taking the first steps to change my behaviour as it’s now obvious that this lifestyle may not for me given that the angst it can cause. Thanks for your assistance and look forward to hearing from you soon. Welcome back to the Forum.  Thanks for continuing confidence in our service.  I'll be glad to comment.  Before I get to the specific questions however- two important generalizations- first, most commercial sex workers, including masseuses do not have HIV or other STIs and, secondly, most single encounters do not lead to infection.  Risk of course goes up with more exposures if a partner happens to be infected.  Regarding your specific questions:

 1. Have I put my health at risk for HIV other STIs from this event.
Acquistion of STIs following receipt of oral sex is rather uncommon.  There are no proven cases of HIV acquired from receipt of oral sex and other STIs are uncommon.  When an STI has been acquired, the STI most commonly acquired is gonorrhea and that is relatively uncommon.  Other STIs are even more uncommon.  90% of persons who acquire gonorrhea develop obvious symptoms within 3-5 days of exposure.  If you do not over this time period, personally I would not worry and would not test.

2. Do I need to test for HIV/STIs from this event and if so how soon, for what and when to test to be conclusive. 
See above.  As Dr. Handsfield has told you previously, some clients choose to test for the added comfort/confidence that a negative test provides.  If you choose to test, the most important test would be a urine test for gonorrhea and chlamydia.  Syphilis and HIV testing would require a blood test and take 6 weeks to provide conclusive results.  Whatever you do, PLEASE DO NOT have a blood test for herpes- the results are not reliable and relatively often give falsely positive results.

3. Do I need abstain for a period of time from normal sexual practices with my regular partner.
This is a personal choice.  I would not feel the need to abstain.

Hi Dr. Thanks for getting back to me and clarifying my concerns. Your expedient response was greatly appreciated. Between my post and your response I managed to consult with a sexual health GP clinic that partners with the main STI clinic in our city. Their professional assessment was very similar to yours and even suggested the same optional std sti testing in 7 days for peace of mind only and they expect negative test results from this encounter. They also stated as I was receiving oral from female the risk was different from giving and if it was M2M. Is that your understanding? Anyway they also stated my encounter was nil risk for HIV, low risk for most sti and like you rather uncommon from receiving oral. Surprisingly they didn’t seem to be that concerned which also helped. Her assessment and your response has alleviated most of my concerns. I’ll just be cautious over the next few days and monitor for symptoms.  Hopefully all goes well. Regards JB

Thanks for the follow-up .  I do agree that if you had received oral sex from a male the risk would be slightly higher than after receiving oral sex from a female.  Either way however, the risk is low.

Just a final follow up as agree with both your and Dr Handsfield I plan on testing in 7 days for that final piece of mind.
The optional STI screen test provided by the local STI clinic GP included for both gonorrhoea and chlamydia. Will results be conclusive for both after then7 days? Thanks and look forward to hearing from you. Regards JB

Dr. Handsfield and I have worked together for over 40 years and we never disagree on the facts we present although our verbal styles.  He agrees with all that I said.

I agree with your plans to test get a urine test for gonorrhea and chlamydia to increase your peace of mind.  I am confident the tests will show that you were not infected.  Results at any time more than 5 days after your encounter will be conclusive.
