[Question #13001] HIV Seroconversion…

Hello,
On April 8, 2025 was when sexual encounter happened… I am a female who had un-protected vaginal sex with my ex boyfriend. Approximately 14 days after encounter I got tested for all sti’s and found to be positive for trichomonisias. Took antibiotics, cleared. Then I went back to test about 1 wk or so after finishing antibiotics since symptoms hadn’t cleared up. Tested positive for ureaplasma… now I’m on doxycycline for that… 4 days into treatment so far. This is just history I’m providing for context. Now to my question. 31 days after exposure, I’ve had 4th gen hiv test done approximately 1 wk ago… test was negative, however yesterday, exactly 37 days post exposure, I started having every symptom of acute hiv. I’m having major body aches all over, chills as I’m very cold, I have a fever of 99.6, 99.7 last I checked which was this morning of typing this. I have very dry mouth and lips, white tongue, lightly stuffed/runny nose every now and then, coughs, extreme fatigue, just sick all around, basically flu symptoms. I’m so afraid and do believe I’m going thru sero conversion. But the 45 days aren’t until Friday coming up. Wondering should I just go to the ER at this point as I’m sure test would show positive by now as I’m having symptoms. Thank you in advance as I know this is a lot but I’m scared to death right now. I could just die.

Welcome to our Forum. Thanks for your questions and your detailed description of your situation. I'll be glad to comment.
I am confident that the symptoms you are experiencing are due to something other than the HIV ARS. There are several reasons I say this. While I know little about your ex, it is statistically unlikely that he had HIV. Even if he did however, your risk for infection following a single encounter would be less than 1 in 2,000 (i.e. there is a 99.95% chance you would not be infected). Further, the HIV ARS most typically occurs 10-20 days after infection and virtually never a more than 30 days after an exposure. If you wish to prove that this is not the ARS, you can test with a 4th generation test at this time- during or after the ARS HIV tests are always positive. I am confident that if you chose to test, I am confident the test will show that you were not infected. It is far, far more likely that your symptoms are due to influenza, COVID, or one of the many other community acquired viral illnesses that we all acquire from time to time.
Incidentally, ureaplasma is a normal component of the vaginal microbiology and very rarely, if ever leads to symptoms. Most specialists would not treat it.

Hi doc,
Thanks for reply. I should’ve added this in initial question… so ex “supposedly”, (I don’t really trust him) since he lies a lot… took 1 rapid test same day of encounter and tested negative, then took another rapid and 4th gen lab test 18 days after encounter… that’s a little history. Plus he was sick with flu or something around march 25 or so… My worry is he had acute HIV when he has sex with me and just too early to show on tests. I will test again because I’m still very scared. It’s just too much of a coincidence for me to get sick like this with all the symptoms of hiv so shortly after encounter.
Also my doc decided to treat me for ureaplasma because I’m having such harsh symptoms like vaginal discharge, pelvic and uterine pain, constant urination with burn… in the absence of any other infections, if the ureaplasma is not causing this, I’m curious as to what else could be causing it. Bull I’ll save that question for my gyn doc…

Thanks for the additional information. Despite your mistrust of your ex, the odds are very much against this being HIV. I'm confident your test will be negative.
I won't get between you and your GYN- it sounds like s/he is doing everything right. I trust you were checked for bacterial vaginosis. EWH

Also how valid is testing with 4 gen hiv testing at 31 days after exposure? Can I have some sort of confidence in it?

At 31 days Fourth generation HIV test testing would certainly be positive if you were symptoms were due to recently acquired infection. If your symptoms are due to something else, a fourth generation test at 31 days would still be more than 98% conclusive.
