[Question #13021] Capillary testing / 4th gen

Dear esteemed Doctors

For reference,  last encounter was in September 2024, receiving unprotected oral from  a TS escort, and me rimming her anus.

I currently have a sore throat which has lasted over 2 weeks now (though getting better). I have NEVER had a sore throat that lasted this long, at max it's a few days. I am worried sick about HIV but unfortunately I cannot sneak out to get tested as it will raise suspicion. I did initially have a bad cough and nasal congestion but now it's just the throat.

I tested negative with 5th gen NHS HIV tests at 2 weeks, 7 weeks, and a private 4th generation test at 9 weeks.

My concern is that these were all home tests done by finger prick, i.e. capillary testing.

1) Are capillary tests as accurate as lab based ones (both 4th and 5th generation)
2) Was my final test at 9 weeks compromised as it was 4th gen and not 5th gen?
3) Are my concerns valid with regard to the long duration of sore throat?

Welcome back to the Forum although I'm sorry you felt the need.  As I said when we last interacted- "A negative test for HIV using a 4th or 5th generation test is conclusive any time more than 6 weeks (42 days) after a possible exposure.  We have never seen or heard of a test becoming positive after that in anyone who has not taken anti-HIV medication. That statement remains true.


The NHS uses approved tests for HIV and in order to be approved tests must perform equally well.  You should believe your results.  Capillary blood tests on finger stick blood are as good as tests performed on blood from a vein.  Thus, in response to your questions: 

 1) Are capillary tests as accurate as lab based ones (both 4th and 5th generation)
Yes, see my comments above.  

2) Was my final test at 9 weeks compromised as it was 4th gen and not 5th gen?
Answered above as well as in the past.  Your results are conclusive.

3) Are my concerns valid with regard to the long duration of sore throat?
You do not have HIV from your September encounter.  You need to move on.  There are many causes of sore throat, even prolonged ones.  I suggest you see your regular doctor about this.  Among other things you could have strep throat which you could pass to family members (which I know is a concern for you).  EWH

Going forward and being realistic, if any further similar interactions were to happen (with TS), I would like to categorise risk.

1) Would rimming be a significant risk for anything significant?

2) If activities were limited to genital-genital frottage, light kissing, mutual mastubation, and maybe rimming, would that negate most STDs apart from the skin to skin STDs?

In the absence of an obvious sore or lesion, no.

Hi Dr

I forgot to mention I had a NEGATIVE 4th gen test at 7 months

Recently my daughter has had a very bad sore throat, fever... this is after my 3 week sore throat.. 

is there any chance the sore throat may be a delayed immune reaction to HIV?

Is there any chance I am someone who tests negative for HIV but has it?

Is my 7 month test enough to say I do not have HIV?

Is there any chance I have infected my daughter?

I'm sorry for the repetitive questions but I'm losing my mind.
