[Question #13049] Concern After Body Slide Massage

3 wks ago I had a body slide massage with SW. She wore a small thong that just covered her labia. I was nude. For 20 mins, she gave a lubed HJ but rubbed my penis against her thong, butt, inner thigh. 20 mins. Showered after. 3 DAYS AFTER: tip of penis felt irritated (no pain while peeing/ejaculating). Tip irritation has remained for 2.5 wks. Bothers mostly when I think about it. Seems to go away sometimes. Have felt some intermittent, urgency to pee. 4 DAYS AFTER: shaved my scrotum. Next day razor burn cropped up . Resolved in a day, but 2 pimples came up in shaved areas. Not painful, no blister/watery discharge/scabbing. 8 DAYS AFTER: had 2 days where i felt really tired, not achy or flu like exactly per se but felt like i was fighting a virus. Appetite was really low (unusual).  I have a signficant other I’m worried about. Anxiety 15/10.Have been wondering if every little thing is shooting pain, swollen groin lymph nodes, tingling. Hard to say. Risk of pyschosomatic, I think.I have GHSV1. An initial OB10 years ago. Unmistakable. The pimples I had concern over did not resemble those lesions. Will test IGG at 6 + 12 weeks, but curious for your risck assessment and if my symptoms feel indicative of GHSV2?

I feel that your risk of acquiring HSV 2 in the situation that you describe is just about zero.  If her thong stayed on and in place, the kind of contact you had with the sex worker does not involve the kind of genital to genital contact that would be required to get HSV 2.
I do wonder why you think the outbreak you had 10 years ago was HSV 1 vs. HSV 2?

Thank you, that's relieving to hear. I got a very self evident herpes outbreak 10 years ago. Serious dysuria and several concurrent lesions. It was swabbed while active and confirmed as HSV1. I know when I got it, after receiving oral sex with a specific person I was with at the time. Have had only one other outbreak (8 years later) and 6 or so years have passed since then. 

The thing that is bothering me here is that the irritation of the tip of my penis, 2 days of malaise, loss of appetite, some urgency to pee and 2 pimples (after shaving... folliculitis) seems like A LOT of stuff going on to be coincidental. I've seen you write elsewhere that loss of appetite and urgency to pee are NOT symptoms of HSV1/2, but it seems to be widely cited as symptoms elsewhere. Any idea why that discrepancy may exist? I recognize genitally focused anxiety may be a source of some, maybe all of this.

I have no idea why people say what they say online - not at all, but there is no biologic reason for either of those to fit with HSV 2 acquisition.  In fact, often people who have a new HSV infection have difficulty initiating a stream of urine or bowel movement as the nerves in that area are impacted negatively.   So that is the opposite of urgency.  And what would lack of appetite have to so with genital infection?  Be careful what you read.

It sounds like if I were your patient you might even not recommend testing in my case? Is that accurate? I will do so because I have a significant other I need to be protective of. But curious. 

Lastly, and then you can close me out, the intermittent feeling of irritation at the tip of my penis, which seems to come and go mostly wtih my focus, that does not concern you? It does not hurt with peeing or ejaculation, it is more of a nagging irritation or sting that has intermittent qualitites.

Sorry, last additional clarification... because the SW was rubbing my penis in her "boxer short region" that doesn't concern? I understand shedding can happen in that region, but your answer indicates the actual vagina/labia area is really what counts? Thank you and appreciate your insight to finish out my line of questions.

Yes, the concern here would be if you rubbed your penis against her labia area or vagina and I don't see you reporting that.  The buttocks and thighs are too thick for viral shedding to occur there.  I'm not sure what the irritation is about here, but I'm quite certain it isn't about an STI if your report of the contact is complete and accurate.  If you actually did get HSV 1 10 years ago, I suppose it could be related to that?
