[Question #13139] Concerned about vaginal to anal transmission of hpv

Hello again !
This time I have an entirely different set of concern with a very unique situation.

About 2 hours ago I visited a massage parlor due to chronic backache. The masseuse undressed and then sat on my buttocks with her vagina facing it for about 2-3 minutes while massaging my lower back. My back which was painful at that time caused by buttocks to be moderately clenched at this time before I asked her to move herself to my hamstring region while she massaged my lower back muscles. So my direct anal and perianal region might not have been "open" enough for a great surface area of contact.


I am concerned about potential spread of hpv from her vaginal contact to my buttocks during this timeframe. Can you please help me understand my situation and concern a bit better. 


Also for the record I had 2 small gw removed from my penis in December without any recurrence so far.

Just to expand a bit her vaginal area probably made contact with my "gluteal region" but not likely the perianal and deeper areas. Or if it did it would be minute at best. Friction was low as there was oil and less movement range and also I don't have any active cuts etc.

Welcome back to the forum.

HPV is rarely if ever transmitted except by insertive sex -- penile-vaginal or penile-anal contact. Genital contact without penetration also can transmit the virus, probably explaining most HPV transmission between women having sex with other women. However, this probably requires prolonged and vigorous genital contact. The brief, superficial sort of exposure you describe is little or no risk. The skin of the buttocks probably is not susceptible to the genital types of HPV. And there would have been little or no risk even if your anal and perianal region had been "open" during this event. You should not be at all worried about HIV fro the event described.

Thank you Dr HHH for the comprehensive response. 

From my understanding the initial "outbreak" of hpv at any site affected would be proportional to the area which was in contact and the length/friction involved. Even if this event would result in transfer of the virus it should not affect a large amount of area ?

I am also unvaccinated and have been sexually active here and there for 8-9 years. Always practiced safe sex. I likely have been exposed to hpv multiple times given my history of prior GW as I mentioned which has resolved. This particular masseuse I know well bad have encountered 5-6 times already since December 2024. Any strain she would have had would likely have had exposed me already. 
Does this information change your initial assessment in any way ?

The response to your closing question is no: none of this changes my assessment or advice. You're overthinking the business about surface area exposed, but in any case there are no data about this.

Having been "sexually active here and there for 8-9 years", you can be certain you already have had one or more HPV infections; at least 50% of persons acquire HPV by the time they have had 3 sex partners and 90% after several partners. The events above made absolutely no change in the likelihood you have HPV or might day have a medical problem as a result. That said, you certainly could consider vaccination, although this depends partly on your age:  the vaccine is advised for everyone up to age 26, and for some additional persons up to age 45, depending on ongoing exposure risk.
