I made a huge mistake and visited a trans csw.  Condom was on me the whole time.  I received oral and gave anal until I finished inside (I was the penetrator).  After the condom still seemed in tact on my penis, there wasn’t leakage of the semen.  I washed my penis/ hands with soap afterwards.  I did ask csw about std status, they said they were clean.  I am having anxiety right now about HIV risk.  I am scared because I do not want to infect my female partner.  What testing would you recommend, how long do I need to avoid sex with female partner?

Welcome back to the forum.

Honestly, if you remember or re-read your two earlier threads -- especially the last two, several months ago -- you probably would already understand the near zero risks here and our response. Condoms are highly effective in preventing HIV and other STIs; and oral sex is essentially zero risk for HIV and some STIs (like chlamydia and HSV2) and very low risk for othereven when unprotected. And people rarely lie about HIV/STI status when asked directly, so your CSW partner probably doesn't have anything to worry about. If somehow I were in your situation, I would not feel a need for testing and would be continuing unprotected sex with my wife with no worry about infecting her with anything.

So I don't advise avoiding sex with your partner and from a medical/risk perspective, no testing. However, we understand that test results often are more reassuring than professional opinion, no matter how expert. (We don't take it personally!) If that applies to you, then after 3-4 days you could have a urine gonorrhea/chlamydia test; after 11 days an HIV RNA PCR test; and after 6 weeks a syphilis blood test. I would expect all these to be negative.

Thank you Dr.  I went back and read, I understand what you are saying.  
1. How definitive is a gon/chlam/trich urine test at 6 days?  
2. Is trich something to test for in this case?

1. Conclusive for gonorrhea and chlamydia. There's been no research on this for trichomonas, but trich is impossible from oral sex; trichomonas does not infect the oral cavity.

Thank you Dr Handsfield for your responses.  I had a few additional concerns.
1. It’s been 2 weeks.  I now notice maybe 2 small bumps on scrotum that resemble ingrowns.  I did exercise/sweat more lately but is there a way to tell if this is HSV?  This area likely came in contact with the inner thigh/ anal area of csw.  They aren’t painful yet, but what should I watch for? 
2.  Does trich ever infect anal area?

No STD causes the sort of skin problems you describe on your scrotum. Your self diagnosis of possible folliculitis (ingrown hairs) sounds plausible. (In general, STDs are among the least common causes of genital skin problems. I have a book titled Genital Dermatology Atlas. Of its 300 pages with innumerable photos and brief descriptions, all STD are covered in 15 pages.)

Trichomonas is not known to infect the anus or rectum.
