Hello again doctors, I feel frustrated, angry with myself and feel like not living anymore, I cheated on my wife yesterday, and I feel terrible about it. The sexual encounter, was with a woman I know but she is quite promiscuous, I performed oral sex to her (cunninlingus) several times, she to me for a few minutes, she wanted me to penetrate her but I did not have enough erection and I could not penetrate her, my fear is HIV, I am very afraid. What is the probability of having it? I could not get the PEP.

First, it probably is unlikely your "promiscuous" partner has HIV, which remains uncommon in women in most countries. Second and most important, there has never been a proved case of HIV transmitted by cunnilingus: you will not be the first in the world. I do not advise PEP; if you came to my clinic, we would not prescribe it.
You really should not be worried, do not need HIV testing, and if you have a regular partner, you can safely continue your usual sexual relationship without putting her (or him) at risk for HIV.

Doctor, I understand what you are telling me. My biggest fear is that I tried to penetrate the woman without protection, my glans was at the entrance of the vagina, and it may be a possibility that my glans was 1 centimeter inside or 2, but I did not have enough erection and I could not penetrate in full this was for about 10 seconds.... I don't know how risky my case is, I really don't know what to think.

This information doesn't change my evaluation or advice. Even with unprotected vaginal sex to completions (several minutes duration), if the female partner has HIV, her male partner has under 1 chance in 2,000 of becoming infected. With the very brief event you describe, I would guess the chance you would have caught HIV as under one in a million even if she has HIV -- which she probably does not, as already discussed.---
