Dear Dr.
I had before several risks that as per your consultation it was zero risk but i had today new risk FROM THE FIRST TIME I HAVE. I visited Half Russian half Iran lady and she gave me oral no condom for 3 min and she sit on me and intercourse happened with condom all the time but it was covered all my penis but the head covered i’m paranoid
NOTE: AFTER THAT I KNOWS THAT SHE IS DOING IT WITHOUT CONDOM BUT NOT ALWAYS
Should i take prep ?
Should i do test for hiv ?
Is this intercourse is zero risk ?
I’m married and my wife is breast feeding our new baby born and afraid to infected her.
Please your help i don’t know what to do ?

Welcome back to the Forum. I'm not clear on your exposure. As I understand it, you received unprotected oral sex (no condom) followed by vaginal intercourse with a condom. Is that correct. If that is the case, the encounter you describe was virtually no risk for HIV. Most commercial sex workers do not have HIV (or other STIs) and even most single unprotected exposures co not lead to infection. There are no proven cased of HIV acquired from receipt of oral sex and condom protected sex is virtually no risk as well.. Unless I have misinterpreted things, my responses to your questions are:
Should i take prep ?
There is no medical or scientific need for taking PEP in this situation.
Should i do test for hiv ?
No, not if my summary is correct.
Is this intercourse is zero risk ?
Virtually no risk- yes
Your wife and child are certainly not at risk from the situation you have described. I hope that this information is helpful. EWH

Yes it was protected the intercourse
So i can resume my sexual life with my wife without testing ?
As i describe the girl i met she is doing intercourse without condom with other people.
Should i relax after this stupid mistake i did or better to get tested for hiv ?

Testing is always a personal choice however if I were in your situation I would be comfortable resuming unprotected sex with my regular partner. some clients in your situation choose to test for the additional reassurance that a negative test provides. If you chose to test, testing with an HIV RNA PCR test will be conclusive any time more than 11 days after your encounter. If you test with a 4th generation, combination HIV antigen/antibody test, results will be more than 98% conclusive at 4 weeks and 100% conclusive at any time more than 6 weeks after the encounter. From medical wise What i understand is no need for testing and i can continue living my life without any fear from hiv right ?
The thing is i see many websites saying there is a risk and test is needed for this encounter that make me anxious .
The PCR test should be RNA or only normal pcr test for hiv after 11 days will be conclusive ?

As you know, this will be my final answer. You have tended to ask repetitive, anxiety driven questions in the past. Our tolerance for this is limited and if you return with further questions, those questions may be deleted without a response or return of your posting fee. This should not be a surprise as you received a similar warning when you were last on the Forum about a year and 1/2 ago.
Medical wise, I am confident that the encounter described was virtually no risk for HIV and from a scientific perspective there is little or no need for testing. Internet statements to the contrary are wrong like much of what is said on the internet.
Most HIV PCR tests are RNA PCR. This is the "normal test". A PCR test taken 11 or more days after your exposure will be conclusive.
