[Question #13212] Herpes Symptoms

Eleven days ago, on the night of 6/28, I had prolonged, vigorous sex with a girl for the first and only time. The next day, on 6/29, I felt subtle soreness and noticed 2 spots on the left side of the shaft of my penis about a 1/2 inch apart. However, I thought nothing of it because I have experienced hair follicles being inflamed after long or less-lubricated sex, and this felt and looked exactly the same. I shouldn’t have, but I masturbated that night. The next day, on 6/30, I used tweezers to pull a hair from the center of each of the 2 bumps.  The bumps are still there today, 11 days after the sexual encounter, and do not seem to be improving. I have not experienced pain from the bumps or any other symptoms whatsoever. I have talked with the girl and she has never had symptoms of an STD and has only had 3 partners, including me, and feels confident she is healthy. Based on this, do you think it is possible that I contracted genital herpes?

Since there was a hair coming out of each of them AND they were not tender AND they should up the very next day, they do not sound herpetic, no.

Thank you for getting back with me. I really appreciate your time and expertise. It seems to be infected hair follicles to me. The bumps still do not seem to be improving. Would you suggest seeing a dermatologist and maybe antibiotics would clear it up? Would you suggest getting an STD test based on this? To be clear, you are not worried about this being herpes? I am pretty OCD about things. I really appreciate you.

I don't recommend a herpes test due to the high rate of false positives, but if you do one, go to labCorp.  They have a new better test with fewer false positives.  But no, I'm not worried this is herpes.

Thank you. Would it be possible to elaborate as to why you aren’t worried this is herpes? Also, if the bumps are relatively the same at day 15 or 16 after the encounter, or just starting to scab, would that make you more confident that it’s not herpes? Would that make it more consistent with folliculitis? Having bumps on my penis after sex is really really worrying me. Thank you so much for your time. What you do for everyone here is amazing.

I think I listed the reasons above that this doesn't sound like herpes to me - mostly because you were able to extract a hair out of each of the bumps - that suggests folliculitis rather than herpes.  Also, herpes lesions wouldn't normally stay the same for 15 or 16 days -- they would start as a water blister, then break open and ulcerate, then scab over, then heal.  This isn't the progress of the bump that you are describing to me.  However, if you are still concerned, you could do an IgG test but two-three weeks is too soon to test - you should wait at least 6 weeks, then a follow up at 10 weeks out.  You could do a baseline test now and if positive, it is more likely from an older infection than this.  You have options here if you are still concerned that this could be HSV.  Just be aware of the possibility of a false positive blood IgG test.  And use LabCorp if you do this.
