[Question #13226] Hiv Scare

Hi need help! My FWB texted me this with pictures morning saying they woke up to a full body rash that spread all over her body and it looks like one similar to what HIV is associated with. I recently got a two blowjobs with no condom from prostitutes. Once on June 13th and the other on July 5th. My FWB and I had condom protected sex around midway through June and unprotected sex where I didn’t cum inside of her on July 1st or 2nd. We also just had unprotected sex last night. I just got an RNA HIV test on July 10th that came back negative. Could I have given her HIV and if I test tomorrow with an RNA test 9 days after my last blowjob would the results be conclusive?

Welcome to the forum. I'm happy to help. I think you're worried about the wrong STDI!
---
HIV is a very rare cause of body wide skin rash and is very rare in females in almost all parts and populations of the country. Is she otherwise at high risk of HIV? Injection drug use for example? Sex with gay/bi men? In addition, HIV almost never causes rash as the main or only symptom. In addition, there has never been a proved case of HIV transmitted oral to penis: even if your partner has HIV, you probably were not at risk. Also tell me a bit more about your sexual habits and practices. Is your FWB your only sex partner recently? Do you have sex with men as well as women?
In regard to STIs, I would be much more concerned about syphilis -- for which rash often is the first symptom. Was syphilis testing done along with HIV three days ago? You really don't need another HIV test tomorrow, but I would strongly advise you to have a syphilis blood test; and same for your FWB partner as well.
Of course there are many causes of unexplained skin rash other than syphilis (or HIV). If her rash continues and syphilis testing is negative, she'll need to see her doctor about diagnosis and follow-up.
I hope these comments are helpful. Let me know if anything isn't clear. And I look forward to hearing your responses to my questions about your sexual experiences and the results of your and your partner's further test results. Best of luck in the meantime.

Hi thanks for the reply! So my FWB hasn’t slept with anyone but me in around 5 months and doesn’t do needle drugs or anything. About a month ago I kinda started a secret habit of seeing escorts which has me extra nervous because I don’t know how commonly they pass STDs/HIV. I guess im wondering specifically if the blowjobs I got could have put her at risk of anything since she randomly got the rash. Is it possible for me to have tested negative on July 10th which is almost 10 days after we last had unprotected sex but still pass something on to her?

oh I also only have sex with women. I saw online and here that the HIV RNA test is conclusive earlier. If I took one today at 10 days after my last encounter would it be conclusive? How long after getting syphilis are you supposed to test?

If your friend's rash is an STI, it seems very likely you are the source. Your negative HIV blood test proves you do not have HIV and could not have infected her -- i.e. reinforces my believe (above) that she doesn't have HIV. However, you and she both must immediately be tested for syphilis. There is no need to way at all -- do it immediately. Everyone with a syphilis rash tests positive (the rash doesn't even show up until someone has been infected several weeks); and if you were the source of infection, your test will be positive. So do not delay even a day. And don't just test yourselves; visit a reliable provider, like your nearest public health STI clinic or other research with good STI expertise.
This thread will be open for one more follow-up comment. I suggest no more discussion until you can report your and your partner's syphilis blood test results; or the diagnosis of her rash after she has seen a doctor and had it diagnosed. OK?

Ok i’m gonna go get tested now. I guess my last questions would just be How likely is it that a woman prostitute passed on Syphilis to me from a blowjob? And how likely is it in general to get any STDs from a blowjob? then finally do you think I should skip another HIV test all together? oh also I don’t know if it makes a difference but the escorts I saw told me they were clean. Thanks so much for the help

As I said, if she has syphilis, there is a chance you are the source of her infection -- but that's only if you have had an ongoing relationship with her, specifically has sex with her more than 6 weeks ago. (But the rash can take up to a year to show up -- so her having other partners 5 months ago still fits with this possibility.) If you didn't have sex with her more than 6 weeks ago and she has syphilis, you are not the source. However, if she caught syphilis elsewhere, then you have been exposed every time you had sex with her over the past few weeks. Syphilis is easily and commonly passed by oral sex.
The other main STIs from oral sex are gonorrhea, herpes due to HSV1, and nongococcal urethritis (NGU) due to oral bacteria. These usually cause obvious symptoms, but it's still a good idea to be tested for them.
It is EXTREMELY important that your FWB partner see an STI experienced clinic or doctor ASAP; secondary syphilis can be very dangerous, on rare occasions even life threatening. She could call her local or state health department for advice about expert STI care in her vicinity.
As I said above, threads normally are closed after two follow-up comments and we're there. However, I'll leave this open for ONE more follow-up and I ask you not do that until you know the results of her blood tests and your own for syphilis (and HIV if you get retested for it). No more comments or questions are permitted until then. OK?

Hello, I took a 10 panel SDT test including HIV RNA and everything came back negative. To tell the truth the girl with the rash actually hasn’t slept with anyone else for probably 1.5 years at this point as we just got out of a relationship and started sleeping together again shortly after in which she adamantly says she hasn’t slept with anyone else.
Does this mean I am in the clear of any STDs? Is an HIV RNA negative 9 days after possible transmission from a blowjob conclusive? Does this also confirm her rash is more likely a symptom of an allergy (urgent care told her that)?
Thanks so much for the help!

oh wait I see I can still ask questions last one sorry but because I tested negative for syphiillis and HIV does that mean I couldn’t have passed it to her? Or can you pass someone something and still test negative for it cause of window periods?

With your testing and your partner's more detailed sexual history, I agree no STD is a likely explanation of her rash. And there innumerable other likely causes, although I can't comment specifically about the urgent care diagnosis of an allergic reaction. I think you can close the book on STD worries. And yes, your negative syphilis blood test is evidence you could not have infected her.
That concludes this thread. I hope the discussion has been helpful. Best wishes and stay safe.
