Greetings Doctor Hook/Handsfield

I am a male and received oral sex (blowjob) for about 15 seconds  from a female that I met while on a trip. I then inserted my finger into her vagina and immediately removed it. I noticed my finger had a tiny lifted piece of cuticle at the tip of my finger above the nail (not bleeding, just looked like dead cuticle peeling up on surface of skin). Does this increase risk? I had an HIV RNA test plus 10 panel STD test on the 12th day (but it was 11 days and 12 hours after the exposure so not full 12 days) would this still be considered conclusive since you guys say “after 11 day conclusive window for HIV RNA?”. All test Came back non reactive. I also had another RNA test and AgAb test at 19 days. Both non reactive and I also had another AgAb on day 29th also non reactive(since I saw about 3 years ago Dr. Handsfield say that the combination of RNA  after 11 days and AgAb after 28 days is considered conclusive). I went to a STD specialist and he said my results for HIV are conclusive and I don’t need more testing. I Just wanted a second opinion. Am I 100% clear of HIV  based on my test results? Do I need more HIV test to confirm? What are my chances of getting syphilis or other STD from this sort of exposure? Gonorrhea and chlamydia results were negative from the first 10 panel STD exam I took. I’ve had a stuffy nose and and a light sore throat/dry throat  for a few days, are these related symptoms to STD?

You overreacted to a zero risk situation. There has never been a case of HIV proved to be transmitted by mouth to penis or by fingering. You didn't need HIV testing at all. And your negative HIV test results are conclusive. Your RNA test at 11 days was all you needed; our previous advice has been modified by new data:  negative RNA at 11 days is conclusive without additional AgAb testing. In other words, I agree exactly with the STD specialist you saw.

Syphilis and other STDs also are rare from such exposures. Presumably the "10 panel" testing included urine for gonorrhea and chlamydia; blood tests for these infections are very poor but negative urine is conclusive if done at least 4-5 days after exposure. The chance of syphilis also is low, but for 100% confirmation you could have another syphilis blood test 6 weeks after the event.

HIV doesn't cause nasal congestions, stuffy nose or cough. You caught a cold, that's all.

So you can relax -- all is well! Let me know if anything isn't clear.

Sorry to bring this up I just want to make sure I understood everything correctly. So based on my HIV RNA at 11.5 Days and HIV RNA at 19 Days (does this means 2 conclusive HIV exams). So Am I 100% clear of HIV? I can move on and stop worrying about HIV? And then what are the chances of my exposure leading to Syphilis? I don’t have any symptoms expect the cold I told you about. And the syphilis test I take at day 42 or after day 42?

I would like to also add my specialist I saw that I mentioned previously, advised me I can go back to having unprotected sex with my significant other based on my results and type of exposure. Could I go back to having unprotected sex with my partner? Or do I wait for the test results you mentioned in your first response?

These HIV related questions repeat the ones above using different words. I haven't changed my mind. You are 100% clear of HIV, which was proved by the first of your two RNA tests; the second was unnecessary.

In-person care by a qualified specialist generally is more reliable than advice from a distant, impersonal resource like this forum. Your STD specialist probably is just as knowledgeable as we are and I see no reason for you to not trust her advice over ours. However, I agree with what she apparently has advised you.

The chance of syphilis from such an exposure isn't quite zero, but it's probably in the range of one chance in millions. If I were in your situation, I would be continuing unprotected sex with my wife without worry, but I also would have a syphilis blood test 6 weeks after the exposure. Yes, you would be taking a minuscule risk of exposing her but that seems preferable to the issues that would come up if you avoid sex for no good reason for several weeks.

Ok and based on the exposure I had. Are there any other STD’s I should test for besides syphilis? Hepatitis, herpes, etc? I already took Urine test for chlamydia and Gonorrhea.  Both negative

I already said my piece about gonorrhea and chlamydia, to which you didn't reply. If you haven't yet had a urine test for it, that would be reasonable -- although as I also said, simple absence of symptoms is nearly 100% proof you don't have it. There is no need for any other testing.

You were at very low risk for all STIs anyway and zero risk for HIV. Ignore any more "what if" or "what do you think" sorts of thoughts. There is nothing you will think of that would change my replies.
