[Question #13309] 30 day post exposure

Hello,
Thank you so much for your assistance with my concerns as I am worried about my recent results.
My exposure was 33 days ago and I received oral from a female and I also gave oral to her ( I am a male ), as well as had protected intercourse.
I took a full STD panel just to be safe at day 21 and at day 30. All tests on day 21 came back negative for both urine and blood tests.
For the test taken on day 30, everything came back negative except for the HSV IGM, which came back positive at 3.14. The values for negative are <0.90 and equivocal is .090-1.09 for positive. YET, the HSV type 1 & 2 IGG both came back negative on the same panel testing. HSV type 2 igg is 0.12 ( negative is 0.51 COI, borderline 0.51<0.99, and positive is 1.00+). For HSV 1, the igg came back at 0.02 ( neg. is 0.60 COI, borderline 0.60<0.99, and positive is 1.00+ ).
To throw a curve ball to the equation, I did get a single cold sore on the corner of my lip right around the day 30 mark. I have gotten cold sores before, and like this one, always just a single sore, never a cluster breakout. And no genital blisters at all.
So my questions are:
Is it safe to assume that the IGM is picking up my cold sore that I've always gotten now and then? But if it is, why wouldn't the IGG tests for either HSV 1 or HSV 2 show in the positive range if I have had HSV of the mouth previously?
And which std's should I retest for at a later window or should I feel confident about the results of the test done at 30 days?
Thank you again so much for all your help!

The IgM test is notoriously poor and unreliable.  I would ignore those results, especially since you've had cold sores in the past.  And the IgG tests are also not great at picking up HSV 1 infection either which is why your IgG test for HSV 1 is negative.
If you are going to do STD testing for clarity on this experience, a routine panel would be fine including things like gonorrhea, chlamydia, syphilis and HIV though I suspect the experience presented little risk.  The testing might be done around 6-8 weeks for greatest accuracy.

Hello Ms. Warren,
Thank you for your response and I will do as recommended by testing again between 6-8 weeks for the full std panel.
For the HSV 2 igg test at week 4, how accurate would you say that result might be...at least 70%? For the HSV 1, you stated that the igg test isn't the most accurate, but I was wondering how accurate the igg is for HSV 2 at week 4 in your opinion.

Thank you again for all your assistance. Having you here to ask these questions is a tremendous help with navigating this period as it is quite nerve racking.

Thank you again!

What I said was that the IgM test is not accurate.  The IgG test at 4 weeks is probably 60% accurate.  Someone who is actually infected with HSV 2 would be positive on the test for HSV 2 about 60% of the time with that time frame.
