[Question #13316] Risk of std

Hello  doctors. 
During sex (under 10 seconds, about 5 thrusts), I initially put a condom on upside down, which stretched it a little, then flipped it to put it on correctly. Not immediately because I was drunk and sloppy. The condom was stretched and  covered the head of my penis but not the base. The new partner  always uses condoms and has no known STDs that I know of. I showered and peed right after. What’s the risk of catching an STD in this scenario? I have a partner in which I don’t use condoms. Just making sure this mistake doesn’t hurt my marriage. Thank you .

An inside out condom is equally effective in STI/HIV prevention; and you are correct in your apparent understanding that condom protection is considered complete if the head of the penis and urethral opening are covered. You also describe a partner who is unlikely to have any transmissible STI. Of course I cannot guarantee this was a truly zero risk sexual exposure, but if somehow I were in your situation I would not be tested for anything and would contintue unprotected sex with my wife without worry.

That said, if you remain nervous or unconvinced, you could be tested for reassurance. If so, have a urine gonorrhea/chlamydia test 4-5 days after the event and blood tests for HIV and syphilis after 6 weeks. This is strictly a suggestion for reassurance, not because of any realistic risk.

Hi dr. Thank you for your quick reply. I actually spoke to my friend today and wanted to update the content and see if you would change your mind:

I had a brief sexual encounter early this morning (around 3 AM) with a friend:

  • I received oral sex for under 5 minutes; no visible sores in her mouth or her genitalia area.

  • Then we had penetrative sex with a condom that was initially put on upside down and stretched slightly, then flipped correctly. The condom covered the head and part of the shaft but did not fully cover the base of the penis. I did check the condom and no break or holes. 

  • The penetration lasted less than 10 seconds, she got on top and I didn’t want to continue so I told her to stop. 

  • The partner regularly uses condoms and has no known STDs; no visible sores were seen.

  • I showered and urinated immediately after.

I plan to test at 10 days with an HIV PCR and urine tests for chlamydia and gonorea. If those are negative are they conclusive? If I don’t develop sores or anything after 2/3 weeks. I can forget about herpes or syphilis . If everything goes well. You think I can resume isex or wait the six weeks. Once again thank you for your help and service

I don't see anything different in this description of your exposure. As I said above, testing is optional but if you go ahead with testing, wait til 11 days after the event It takes that long for a negative HIV PCR to be conclusive.. Gonorrhea/chlamydia testing is valid after 4-5 days and will remain valid at 11 days.

Or speak with your new partner and go together for testing now. You might find she also is nervous about the event; in general women are at greater risk from their male partners then the other way around. If you both have negative results on these tests,  you'll both know you were not exposed and can immediately have sex with your regular or other partners. (If you do this, consider including syphilis blood tests as well.) Also, with this plan there is no need to wait to have sex with your regular partner.

Oops, I should not have responded to this question, and you should not have sign in with a different new email address in order to get around the forum policies about repeat anxiety driven questions. Multiple accounts are not permitted. You have repeatedly asked versions of the same questions for more than three years. Your previous thread thee months ago ended with this:  "You previously have been advised about the forum policy against repeated questions on the same topic.... This time the warning will stick:  future questions along these lines will be subject to deletion without reply and without refund of the posting fee. ASHA is not keen on collecting payment when the answer is repeated and predictable, and such questions have reduced educational value for other users, one of the forum's goals."
