[Question #13359] STD exposure unprotected oraland protected vaginal and handjob

Dear Doctor,
I feel very ashamed that I did so regretful thing. I still did not believe how I was so stupid to do this. In Angola on 12 and 13th of July I had contact with to hotel sex workers. Two diffrent one in diffrent day. I received unrpotected oral sex, short vaginal intercourse with condom and handjob without condom. Condom was not broken but remoevd before handjob. Same situation in both occasions. Since then I was checking my penis looking for some symptoms and about two weeks later I have experinced something like balantitis symptoms. I have to admit that I was excesivlly inspecting my penis and washed with liquid soap my glans. When I stoped to used sope symptoms of balanitis didn't totaly gone but conditio significiently improved. I would add that since 11th July I'm taking 100mg Doxy as Malaria prophylaxis. Would you be able to assess risk of STD and advise what symptoms I suppose to look? I won't be able to test myself erlier then in next week. I appologize for my english as it is not my first lenguage and I'm extremly anxious. Furthermore I am so disapointed of myself.

Welcome to the Forum. Thanks for your questions and your implied confidence in our service. The exposures you describe was relatively low risk for STIs. The doxycycline you took may have had several effects including virtually eliminating any risk for syphilis or chlamydia and reducing your risk for gonorrhea. It may also have predisposed you to fungal balanitis. Receipt of masturbation is a no risk activity and receipt or oral sex as well as condom protected vaginal sex are low risk for most STIs and virtually no risk for HIV as long as the condom was worn throughout the exposure and, like yours, did not break. It is unlikely that you have any STI from these exposures but testing is reasonable. When you are able to test, I would suggest blood tests for HIV and syphilis as well as a urine test for gonorrhea and chlamydia. If the balanitis is still present it should be examined by a trained clinician. As I've already said, the risk that you are infected is low but it sounds like testing is reasonable and will provide reassuring results. I anticipate that the tests, all of which will be conclusive at this time, will show that you were not infected.
