[Question #13411] Massage risks

Hello Doctor. Over the past year I’ve gotten 10 or 15 happy ending massages. These were all done only with oil and only hand contact, never anything more. The last time was a month ago. I had not done it for a few months and slipped up and immediately afterward applied a lot of hand sanitizer all over my penis, including the end because I felt like she was a bit rough with the tip.

About a 4-5 days after that experience a month ago I started to notice a lot of discomfort on the end of my penis and also some discoloration almost like brown or gray freckles that maybe I hadn’t noticed before. This discomfort peaked at about 2 to 2 1/2 weeks and has slowly gotten better but is still not gone after a month. About five days ago, I noticed in the shower a very small bump in the pubic hair at the crease of the thigh. I thought maybe it was a skin tag or is it? I think it was just flesh colored and I tried to squeeze it. Became red and I’ve been putting Neosporin on it for a few days now. It looks like the inflammation is going down, but I think the bump is still there. I’m concerned that it could be a genital wart.
At seven days after the massage I went to a local InstaCare have the doctor look at it and give an STI panel. Everything came back negative. But I know that wouldn’t catch everything.
I’ve been married for over two decades and I’m very concerned about all of this and my stupid behavior, and the impact this could have on my life. I realize the risk is rare, but I did this several times unfortunately all in the last 10 months or so. I have not been intimate with my wife since this last time because I’m terrified.

Welcome to our Forum.  Thanks for your questions, as well as your implied confidence in our service.  I'll be glad to comment.  Don't beat yourself up over your massages over that past year- you did not put yourself or your wife at risk.  Receipt of masturbation is a no risk activity with no risk for any STI, including HPV/genital warts.  

We find that it is common for clients who have had an encounter that they have come to regret to both examine themselves (sometimes vigorously) and to have a heightened awareness of otherwise normal findings which had previously gone unnoticed.  Application of irritants including alcohol containing hand sanitizer and Neosporin can also cause irritation on their own or increase irritation due to vigorous masturbation.  I suspect that this is the case here.  

The "bump" you noticed is in a location where skin tags are quite common and warts, which typically take 2-3 months to develop, are quite rare.  I am confident that the lesion you have found has been there for some time- such things do not develop over a few days or even weeks.

I hope this information is helpful.  I see nothing to be concerned about here.  Please don't worry.  If you feel the need, a visit to a dermatologist will verify that the skin tag (most likely diagnosis) is not a genital wart.  EWH

Thank you for that reassurance. I do feel immense regret for what I’ve done and the risk I’ve incurred. I should mention I am age 50 and I don’t believe my wife or I were ever vaccinated for HPV. There were encounters that had happened since last fall, so I wondered if that could’ve been a wart forming from previous times, not this last time only a month ago. Would it be normal for my penis to still feel a bit off from the hand sanitizer a month ago? I am noticing the discomfort is really inside the tip of the penis. Nothing really on the outside.. 

Do you believe I’m OK to be intimate with my wife at this point? Do you think I need further testing?Thank you.

HPV is not transmitted by hand to genital contact.  Further the warts that persons get on their hands (and feet) from time to time are caused by different HPV types from the types which cause genital infection.

Irritation from hand sanitizer or Neosporin should only last a few days, at most.  I wonder if you have been repeatedly examining yourself.  If so, along with your owned heightened awareness, this is a possible source of your continuing irritation.

I can think of no reason to abstain from unprotected sexual contact with your wife related to anything you have described here.  I see no need for testing at all.

A few more things to ask so I can put this out of my mind. 
- you didn’t comment on the mild discoloration on the gland of my penis. Almost looks like freckles or streaks of light, gray or brown. Not raised and they’ve looked the same since I noticed them a few weeks ago. A few days after I applied the hand sanitizer I went on a long hike and that’s when I really noticed a lot of rubbing and irritation, and it seemed to grow from there.
- I don’t know if I’ve been rigorously checking but I have been looking at my penis and the underside of it once or twice a day. It’s just feels a little bit off inside the pee hole, but doesn’t hurt when I urinate. I have been avoiding masturbation, almost entirely.
- I am recollecting one encounter that happened at a massage with hand and oil only where a day or two before I may have nicked my scrotum shaving. Does this increase my risk of anything? This would’ve been seven or eight months ago. It would be below the base of the penis on the scrotum and not near tiny skin tag in the groin pubic area.

Sorry to be persistent with the questions. I am quite stressed about all of this.

And perhaps the lymph node in the groin is slightly enlarged? Not sure if I’m checking it properly.

These will be my final responses- then the thread will be closed.  There really should be no reason for you to return with further questions about your no risk encounters.

There are no STIs which would cause the discoloration you describe.  It sounds like they have not changed since you noticed them and if there is no associated inflammation, this is likely to be something that you have had but not noticed in the past.  I am confident that it is not STI- related.  If, other than you concern about STIs, you should see a dermatologist- there are literally hundreds of different dermatologic conditions which can occur in the genital regions.

Having nicked your scrotum while shaving does not increase you risk for STI and does not change the fact that all experts and organizations such as the CDC and WHO unanimously agree that receipt of masturbation does NOT put you at risk for STI.  

That you have found what you think may be an enlarged lymph node (which is a normal finding in most people unless it is changing and/or tender) suggests that you are checking yourself more than you have in the past.  I would not worry that it is STI related.
