Hi
A woman I was with was fingering herself and then put her fingers in my mouth. We didn’t have intercourse or oral sex with each other.
1) Can I (a male) get gonorrhea or chlamydia in the mouth from that?
2) if so, can you transmit oral gonorrhea or chlamydia to another woman’s vagina?
3) if a woman isn’t planning on having children anymore what implications would those to infections have on them?

Welcome back to the Forum. Thanks for your continuing confidence in our service. I'll be glad to comment. Th events you describe did not put you at risk for gonorrhea, chlamydia or HIV. Transfer of genital secretions from person to person on someone's hands and fingers virtually never results in transmission of infection, if your partner had to be infected (which is statistically unlikely)

Theoretically, a person with oral gonorrhea (or chlamydia which very rarely infects the throat) could transmit their infection to a woman through cunnilingus but the frequency of this event is close to, if not zero. I have never heard of oral gonorrhea or chlamydia being transferred to a partner through cunnilingus.
3) if a woman isn’t planning on having children anymore what implications would those to infections have on them?
In women who develop complicated infections due to gonorrhea or chlamydia (pelvic inflammatory disease -PID) scarring due to the infection can cause infertility, increase risk for tubal pregnancy and chronic pelvic pain. If the woman is not planning to have children, there is still a risk of chronic pelvic pain due to scarring. As noted above, this is not a concern in the situation you describe.
I see no reason for concern regarding the exposure you describe and no reason for STI testing. EWH

Thanks very much for answering.
1) I wanted to run this by you, I’ve been with my GF for a year and I was tested for HIV at 9 months in. We don’t use condoms. She hasn’t been tested since we’ve been together. I think there was one BF between her last test and me. We are in a long term relationship. If she had HIV would probably have been transmitted to me by the 9 month mark?
2) I also had fungus on my penis which my urologist said wasn’t an STD. He said from masturbation or moisture. I am circumcised. Should I be concerned about that? I read online fungus could be from being immunocompromised such as HIV or diabetes. How common is that? Google gives no context it just lists things.

1. With unprotected intercourse, HIV is transmitted on average, about once in every 2500 sex acts so it is possible that, if she had HIV which sounds unlikely, the infection would not have been transmitted to you. At the same time, it ir rather unlikely that she has HIV
2. Anyone can get a penile fungal infection, it's just that persons with diabetes or HIV are more likely to get them. Unless you've had trouble with treatment, I would not worry about your penile fungal infection- just get it treated. EWH

Lastly,
1) I got tested for HIV and Syphilis with other blood work at a regular physical. Both came back negative. However, 7 days later i noticed a lump on the lower end of my shaft where my penis connects to my body/scrotum. Surrounded by pubic hair. It’s a lump, not tender or oozing. Looks a little red. I haven’t shaved at all…….If that were a syphilis chancre would my test be positive 7 days ago? (Btw Dr thought it was cyst from hair follicle)
2) following up on my previous reply, if a couple has unprotected sex for a year and if one has syphilis how likely is it the other partner would get it in a year’s time?
2A) would syphilis probably be transmitted by a year via unprotected straight sex?
3) how would a hair follicle/cyst differ from a syphilis chancre?
4) i was given a cream for the cyst and 100mg of Doxy twice a day for 7 days. Would that cure syphilis as well if I had it?
4) lastly should be concerned at all?

Final responses:
1. Lesion you described sounds nothing like an STI including Syphilis. It is also in a location where Syphilis lesions rarely if ever occurs. As your doctor has already told you, it is in an area where folliculitis is common and what you describe would be the typical presentation of early folliculitis.
2. The transmission of Syphilis is dependent on whether or not there is direct contact with the lesion and how many times that contact occurs. Estimates are that Syphilis is transmitted by lesion contact about 25 to 30% of the time following a single exposure. Obviously multiple exposures increase the risk of transmission
3. Absolutely. A Syphilis chancre is an open sore and in 80% of time Syphilis chancres occur on the penis shaft or head of the penis. Folliculitis on the other hand occurs in areas where body hair or hair follicles are present and starts with a red somewhat tender bump, which can grow. Overtime folliculitis may form a pustule ( pimple) which can break open.

4. Yes, seven days of doxycycline taken twice a day would cure Syphilis. The recommended duration of therapy is 10 days, but seven days would almost certainly be effective. Doxycycline would also potentially cure folliculitis.
5. I would have no concerns whatsoever.
