[Question #13533] My HPV story + questions

Male, 33, history of genital warts.
1st wart back in 2020, treated with cryotherapy, it recurred + bad post-treatment care (lots of nicks while shaving, the warts spread). From 2020 to 2025 I stopped treating them, they sometimes changed color to a lighter tone but never disappeared. Also, I smoked during all this time, which might not have helped (although saw from prev. answers you don't necessarily agree).

Fast forward to 2025, decided to treat it seriously:
- April: Had 5-6 warts (5 at the base of the penis, 3 of them in a cluster, 1 on the penis itself), got them all cryo-ed + got Gardasil-9 1st shot
- June: 1 in the cluster recurs, cryo-ed again 
- August: 1 in the cluster recurs again, cryo-ed
- October: Same thing, treated with pulse dye laser + got Gardasil-9 2nd shot
- No recurrence since then (still early, I know)

Quit smoking since June, multi-vitamins + starting inosine pranobex (3g/day, 28 days) - some studies suggest improved clearance rates, do you agree? Any other suggestion?

Welcome to our Forum.  Thanks for your questions and your confidence in our service.  You are asking good questions.  I'll be glad to comment and share what is known.

You are doing everything right.  The critical issues here are, 1.  How likely are your warts t return and 2. Are there data on what steps will help to reduce the risk of recurrence.  

1.  Following therapy such as cryo, laser or excision, 20-30% of warts will recur, typically within 2-3 months of resolution.  This is because infected cells extend in the skin below the visible potion of the wart and can regrow.  Later recurrences certainly do occur but the longer the interval since treatment, the less likely there is to be a recurrence.  There are few data on the topic or recurrence beyond that.  The numbers of recurrences you have experienced but not unheard of.

2.  There are limited data that HPV vaccination reduces the likelihood of HPV recurrence but the data are sparse and not definitive.  Vaccination leads to higher levels of anti-HPV antibodies than matura infections and that has been suggested to be the reason for the reduced recurrence rate.  

These are the data.  I hope the information is helpful.  You are doing what's the best you can do.  Only time will tell whether your warts will recur or not.  Should they recur you might consider seeing a clinical immunologist to determine of there is an immunological explanation for the tendency of your warts to recur.   Sorry I don't have better answers.  EWH

Thanks for the answer and reassurance that my methods are right
A few follow-up questions:

1. Since back in April I had 6 warts spread across my pubic area and currently only have 1 at a time (unsure if it's always the same one as it was a cluster), is it right to assume the body is doing well in clearing the other local infections? It's been >6 months since any of the others recurred.

2. I quit smoking back in June + started taking vitamins and asked the doctor treating my warts if stopping alcohol (just a weekend drinker) was gonna help. He said ceasing tobacco is more beneficial and social drinking is completely fine. What's your take?

3. A difference I am noticing after the last laser treatment is I barely have any itching/tingling sensation, something I always had after past treatments, usually when the warts were about to recur. Is itching/tingling a symptom?

Thanks for the follow up information. Your history is rather unusual and the location of warts in the pubic area is atypical. Are you sure these are warts? If you’ve not already seen a dermatologist my advice before undergoing more HPV therapy is to see a dermatologist for a definitive diagnosis. In the interim, in response to your specific questions: 

1.  Please see my comment at the above. The more details you provide the more I find myself wondering whether these lesions are actually warts or not. I recommend you see a dermatologist for evaluation and management.

2.  I am unaware of data that cessation of smoking and social drinking, while perhaps good for your health in general, has a significant effect on the tendency of HPV infections to recur.

3.  In addition to the atypical location, it is unusual for warts to be associated with a specific sensation.

As I have said above, I think it is time for you to get a second opinion on what these lesions that are trouble you might be. I wonder about other dermatological problems, including molluscum. EWH.
