[Question #13545] HIV risk

Very nervous about exposure. Had repeated and prolonged touching and massaging of outside of rectum /anus at a massage parlor by Asian female mid 40s in NYC massage parlor on 10/21/25. No gloves used
Dark room unable to see if any visible cuts on fingers or hands
Extremely distressed concerned about HIV risk. Had DUO 4th gen tests —1 week and 2 week post exposure. —both negative
What is risk ?
Will a defintive test be at 3 weeks ? Is three week tests accurate
Does this warrant further testing ? Concerned about partner who knows about exposure

Welcome to the Forum. Thanks for your questions and your implied confidence in our service. I'll be glad to comment.
You describe masturbation without other sexual contact- this is a no risk activity, no matter whether your partner had genital secretions or cuts on her hands. Scientifically there is no reason for concern and no reason for testing.
While there is no reason for testing, I realize that some clients appreciate the added confidence that negative tests provide. If you are such a person, your 3 week test would detect about 50-60% of recent infections. At 4 weeks a 4th generation HIV test would detect at least 98-99% of recent infections and would provide conclusive results any time more than 6 weeks following your encounter. If you feel earlier conclusive results are needed, an HIV RNA PCR test would detect all recent infections any time more than 11 days after the encounter. As I said, there is no scientific reason for testing.
I hope that this information is helpful. EWH

Dear Dr Hook
Thank you. This is extremely reassuring.
Just so that I am clear, regarding my original question, the person being massaged was naked and the massage therapist repeatedly touched and massaged the anus and rectum, with direct contact, no gloves or barriers.
Even with that type of contact, this is considered a no risk situation? (Unaware if she had cuts or abrasions on hands?)
If such is the case, based on your response, then no further testing is even warranted?
Incidentally, a full STD panel was done at 3 weeks, including HIV duo—all negative
I just wanted to make sure that my question was clear for the most accurate response.
I sincerely appreciate your reassuring words and response.
As you can imagine, I’d like to move on with my life with my future wife and put this incident behind me.

Thanks for your clarification. Your description is precisely what I presumed. My assessment and recommendation is unchanged. I see no need for continued testing or concern. EWH---
