Greetings to my respected doctor. Sorry for using translation because I'm from Indonesia and don't understand English. I'm a married man with one son who is still breastfeeding from my wife.
I've now made the wrong decision and have fallen into regret. I had sex with a street prostitute. I don't want to infect my wife and son with something as horrible as HIV.
What I did was the woman grabbed my penis and put it in her mouth without a condom, maybe only for about three sucks because I stopped her. We then had sex with a condom. What worries me is that I wasn't focused on the process of putting on and removing the condom after intercourse because the woman did it.
What I remember is that after I finished having sex, my sperm was still on the condom. The sex didn't last long, maybe no more than 2 minutes, and perhaps the woman didn't ejaculate much because she was aroused. Does the duration of sex and the lack of fluids make me less likely to be exposed? Today, I failed to get a PEP from a government doctor, and I don't have access to the city because a specialist sex clinic is only an 8-hour drive away. I'm very worried. I hope you can give me some advice on what I should do now to make sure everything is okay. Thank you

Welcome to our forum. Thank you for your questions and for your confidence in our service. I’ll be glad to provide some information.

The risk of HIV from the encounters you describe is Low. HIV is relatively uncommon in Indonesia.  Most commercial sex workers do not have HIV and even when they do, unprotected penile, vaginal intercourse leads to acquisition of infection, on average, only once in every more than 2000 encounters. In other words, if your partner did have untreated HIV And the condom was not used, there is a more than 99.95% likelihood that you would not be infected. The fact that you use a condom makes your risk of infection even lower.  You can have confidence that your condom did its job. That you saw your ejaculate in the condo after sex proves that it worked. When condoms fail, they break wide open, leaving no doubt that they have failed.

There are no proven cases of HIV, which have been acquired through receiving oral sex. That she put your penis in her mouth is not a risk factor for HIV infection.

For a medical and scientific perspective, there is really no need for concern and very little reason for testing. On the other hand, testing is a personal choice and if you choose to test, a fourth generation HIV blood test will detect over 98% of recent infections when taking four weeks after your encounter and at six weeks would provide entirely conclusive results.

Thank you for your answer. For your information, I was tested for HIV 5 months ago with a non-reactive result. Do you strongly recommend that I get tested again now? Do you think I'm putting my wife at risk if I have sex with her without a condom?

Testing is always a personal choice.  Given what you have described, I would not feel the need to test.  There is little reason to worry that youmight be putting your wife at risk.  EWH---
