[Question #13611] Unprotected/“Protected” Oral

Hello Doctors, about 4 weeks ago I had unprotected oral and protected intercourse with a CSW. 

I got tested for Gonorrhea and Chlamydia via a urine sample two weeks after the incident.

Yesterday I had “protected” oral where the CSW was in a massage parlor and used what looked to be two regular store bought plastic bags as a “cover”. 

My question for the second incident is whether this wouldve actually provided any protection at all or if you feel I should be tested again before resuming sexual activity with my significant other. 

Also with both incidents in mind would you feel the need to get a blood test for syphilis/HIV at the 6 week mark? And if so is it necessary to start the 6 week timeline from the most recent exposure or will a test 6 weeks from the exposure a month ago cover my bases?

Welcome to our forum. Thank you for your questions. I’ll be glad to comment and will do my best to provide information. If something other than a real condom was used during your receipt of oral sex there are no data to inform how effective that might be however, it is logical to presume that it did provide effective protection. When you combine that with the fact that most commercial sex workers do not have STI‘s, that oral infections are less common than genital infections, and that not every exposure to an infected partner results in transmission, it is quite unlikely that you were infected. If no symptoms develop, I would not feel the need for testing. 

Likewise, I do not feel that testing for syphilis and HIV before returning to unprotected sex with your regular partner is essential. There are no proven cases of HIV acquired from receipt of oral sex and Syphilis is far more uncommon than other STI which you have tested for such as gonorrhea and chlamydia.

My advice to you is to move on from this encounter and to not worry. I see no need for testing. EWH.

Thank you for the reassurances doctor. So based on my understanding, although it wasn’t legitimate protection, due to the reasons you described as long as I dont show obvious symptoms of gonorrhea and chlamydia (the two most common Oral Transmitted STIs) within the next week I should be in the clear.

Secondly, I understand your statistics regarding oral transmission of syphilis and HIV after going through similar questions in the forum. 

Would you say that since the intercourse in the previous (3 weeks ago) incident was protected by condom usage my chances of syphilis/HIV are negligible? I will most likely take a full run of STD tests at the 6 week mark from the intercourse as I try to get tested every six months. If I wanted to be extremely sure about the blood tests being accurate would you advise me to wait until 6 weeks from yesterday? Or is 6 weeks from the previous incident long enough to be conclusive even for this newer “exposure”.

Lastly, if I resume sexual activity with my significant other, as long as there are no chancres I cannot pass on potential Syphilis correct? Information on the internet seems to be 50/50 on whether you can transmit without a chancre. Based on responses in this forum it seems that transmission solely relies on direct contact with a chancre.

I agree with your summaries and like you, presume you are in the clear.  To be entirely in the clear, I’d wait until 6 weeks following your last exposure.  In the interim I would not worry about resuming unprotected sex with your regular partner.  EWH---

Sounds just to confirm you mean wait until six weeks from this most recent “protected” exposure to fully be cleared by the full run of tests. 

Secondly, regarding continuing sex with my partner Gonorrhea and Chlamydia are pretty overt in symptoms so I would notice within the week and abstain from sex in the case they do show.

Based on other replies in the forum Syphilis is not as common as it is made out to be, especially in places like the US with substantial medicinal standards. 

My questions is that as long as there are no chancres I cannot pass on potential Syphilis correct? Information on the internet seems to be 50/50 on whether or not you can transmit without a chancre. Based on responses in this forum it seems that transmission solely relies on direct contact with a chancre.
As long as no chancres are present I can safely and confidently resume sex with my partner.

By “protected” exposure I mean the plastic bag oral. So 6 weeks from two days ago. As 6 weeks from the previous exposure (protected intercourse and unprotected oral approx. 3-4 weeks ago) wouldn’t be conclusive on any potential new concerns from the plastic bag oral 2 days ago?

Sigh.   Final responses to these repetitive questions.  The thread will then be closed.  Please do not return with further questions on this activity.  If you do, the question may be closed without a reply and without return of your posting fee.

1.  If you need a negative test to move forward, 6 weeks after your last possible exposure results of a 4th generation test for HIV will be conclusive, as will tests for other STIs such as syphilis.

2.  50-60% of men with penile chlamydia will be asymptomatic.  The figure for gonorrhea is 10%. A urine test will provide conclusive results for gonorrhea and chlamydia will be conclusive at 3-5 days following your last potential exposure.

3.  Correct  regarding syphilis- it is relatively uncommon.

4.  Syphilis is transmitted through direct contact with infectious lesions such a chancres.

5.  I have already commented on resuming unprotected sex with your regular partner.

6.  If you are concerned about receipt of oral with a plastic bag over your penis, you should wait until 6 weeks after than exposure.
