[Question #13665] Risk of oral transmission

Good evening,

I received oral sex from a hooker in the US, I wanted to know you think my risk of being infected, so the oral sex was unprotected its was brief not lasting more that 1 min I ejaculated in her mouth and she never deep throat or left alot saliva on the penis. Its been over 3 weeks I haven't seen any symptoms maybe the random urthera irration but no discharge. I am not really worried for HIV or Hepatitis since its extremely rare to be transmitted this way.


1) what are my risk for Chlamydia, gonorrhea and Mycoplasma genitalium or syphilis ( i didnt see any sores on her mouth)?


2)If I was your patient  would you want me to do a skin for this std from this particular exposure 

3) in the past I had Chlamydia like 8 years ago it was symptomatic(i did received medical treatment), if I was symptomatic back then could I be asymptomatic now?

Welcome back to the Forum. Thanks for your continuing confidence in our service.  I'll be glad to provide some information.  Your exposure was low risk- most CSWs do not have STIs and even when they do, most exposures do not lead to transmission of infection.  Of all potential penetrative exposures, receipt of oral sex is amongst the lowest.  There are no proven cases of HIV acquired from receipt of oral sex and as you point out, hepatitis is likewise extraordinarily rare, if it happens at all.  The most common STI acquired from receipt of oral sex is gonorrhea which is obviously symptomatic with a penile discharge within 3-5 days of exposure.  Chlamydial infections is quite rare and syphilis is likewise very, very uncommon.  That 3 weeks has passed without obvious symptoms makes the likelihood that you were infected virtually zero.   I would not worry about your intermittent discomfort on urination- when STIs cause symptoms they are continuuous, not intermittent.  In response to your specific questions;

See above.  There are no data on the acquistion of M. genitalium as well and I would not be concerned

2)If I was your patient  would you want me to do a skin for this std from this particular exposure 
Testing is always a personal decision but in your case, at this time, I would not bother to test.  Should you opt to test, far and away the most important test to get would be a urine test for gonorrhea.  I would not bother with blood tests of any sort

About half of chlamydia in men is asymptomatic.  As i said, chlamydia is rarer (about 10 times less common) as an oral infection than gonorrhea.  If you opt to test, typically chlamydia and gonorrhea are tested for in the same urine specimen.
