[Question #13674] Massage

Hi drs, I had a massage with a trans escort 6 weeks ago. I totally get low risk. Don't think she even touched my penis I finished myself off to get out quickly. Only reason for concern is her penis grazed my bum a few times, and while looking for a chancre, the last week or so all I can see is a load of red small dots so just want to make sure that's not unusual? Two big spots which just look like spots, and loads of spots which I assume is folliculitis  but I'm panicked about syphilis. Would red spots be a symptom of secondary?

I'm glad you understand the low risk nature of the exposure on your mind. I can further reassure you that the brief, fleeting contact of your massage partner's penis with your anal area carried no risk of syphilis or any STI. In general, for infection to take hold, STI pathogens must either be deposited inside a body cavity (vagina, rectum, mouth -- mainly gonorrhea, chlamydia, HIV); or must be vigorously massaged into the tissues (herpes, HPV, syphilis). That's why chancres, initial herpes symptoms, and most genital warts occur at sites of friction during sex -- penis head, vaginal opening or labia, near the anus, etc.

In addition, neither syphilis nor any other STI causes "a load of small red dots". My best guess is that nothing new is present at all -- that is I wonder if you're noticing minor skin changes or trivial hemangiomas that have been there all along. As for possible folliculitis, that would show up as inflamed enlargements around hair follicles. In any case, I'm confident they have nothing to do with the massage exposure 6 weeks ago. However, of course you are free to get tested for syphilis if the negative result would help reassure you; 6 weeks is enough time for conclusive blood testing. Finally, if you are confident these various skin changes are new, see a doctor (ideally a dermatologist) to sort things out.
