[Question #13757] Insertive Oral Sex

Hi Doctors,
Back again just for a small query. I was talked into having oral sex with a gay person that I have known for a few years in the gulf. He is very sexually active so I asked him if he was hiv negative, he said he was on prep and he had tested for everything a week and a half ago and everything was negative. The oral sex lasted a minute and I came in his mouth. After a few days I got a burning sensation at the tip of my urethra that would come and go. Feeling a bit worried I got tested for gonnorhea and chlamydia on the 6th day and it came back negative. But the burning sensation is still there after 11 days. It comes and goes and hasn’t got worse. Sometimes when I shower it gets painful. But not burning when I pee. Any idea what this could be? Thanks

Based on previous advice, I had unprotected sex with my wife after 8 days as there was no risk of chlamydia or gonnorhea.

You already know we will advise there is no risk of HIV transmission mouth to penis. On top of that, your oral sex partner could not have HIV for all the reasons you describe.

Hypothetically you could have been at risk for gonorrhea, nongonoccal urethritis and herpes due to HSV1. However, none of this and no other urethral infection can cause symptoms that appear and disappear ("come and go"). That fact plus the absence of visible discharge or painful urination all point to anxiety elevating your perception of trivial symptoms or normal body sensation that otherwise would not be bothersome and that you might not even notice. Probably you have not put your wife at risk for anything. However, for reassurance you might wish to have a urine gonorrhea/chlamydia test. You also could tell your oral sex partner about your symptoms and ask him to be tested for oral STDs. In the meantime, if your symptoms continue, see a doctor or clinic. But in the meantime I see no cause for worry. We are not a resource for direct medical care.

Yes, I had a urine test at day 6 for chlamydia and gonnorhea and was negative. Do I need further testing?

Just to add, the test was NAAT. Are the results conclusive? Or should I get docycyline as a precaution. Not worried about HIV. Just Chlamydia, the burning at the tip of the urethra is bad today but doesn’t burn when urinating. Slight pains in the pubic region as well.

No comment because it made no difference in my evaluation or advice. NAAT testing is conclusive; you should not take doxycycline.---
