[Question #13784] Daily prep and condom hiv fear

Dear Doctor 
I am male heterosexual uncircumcised in Delhi , India
I am on daily prep ( generic Truvada) never missed a single dose .I kept record for it so 100 percent sure 
Prep daily start on 9th Aug 2025
1. Exposure 6th Dec 2025 after around 4 months of daily prep.....massage parlor ......I do cunnilingus on her but lick only clitoris.didnot taste blood but can't confirm.....I get unprotected blowjob and analingus and .condom protected vaginal sex for 1 minute only  ........little blood on condom after sex but condom remain intact no break  no slip confirm using water test 

2. Exposure on 20th Dec 2025 unprotected blowjob by same women and protectecd vaginal sex for 1-2 minutes 
But when I withdraw  my penis   condom like stuck in vagina but come out and  condom slip upwards but still cover head of penis it's like half on penis and half hanging with semen in it .........so I think head is always covered even during withdrawal.
 I asked her about hiv test she said she is negative showed her full report 6 months back hiv negative from June 2025......
I contiue prep for next 42 days after exposure and stop on 25th Jan 2026 never missed a dose ......
I don't have any symptoms 
My questions .
1. Since I am on daily prep and my condom use ( even little slip but head covered ) and her hands with vaginal fluid on it touch condom too . My exposure is condom protected or not and am I safe from hiv .....
2. Do I really need to test for this exposure .
3. I have a feeling that she has high chance of hiv positive worried me do prep really protect even if condom fails.
4. Do cunnilingus with little blood can transfer hiv ....will daily prep protect me from hiv .

Welcome back to the Forum.  I'm sorry you continue to be so fearful of HIV despite the very effective precautions you describe.  Your partner said she had been tested and was negative- most people tell the truth.  Further, HIV is almost never transmitted through receipt of oral sex or cunnilingus, even if a small amount of infected material, blood or secretions is ingested.  Finally, if the condom remained covering the head of your penis as you withdrew, it did it's job.  It is not uncommon for condoms to slip down or even remain partially inside a partner's vagina of withdrawal following intercourse.  This:

1.  The was safe, virtually no risk sex.  The condom did it's job, you do not know that your partner was infected, and you are on PrEP which is more than 99% effective in preventing HIV if exposed.
2.  I see no sceintitfic reason for testing related to the events you describe.
3.  See my comments above, even if she had HIV (you are not very trusting) the PrEP would have almost certainly prevented infection.
4. Cunnilingus almost with an infected partner almost never leads to infection, particularly since you are on PrEP.

I think you are worrying unnecessarily.  EWH

I have anxiety related to hiv but after taking daily prep i feel relaxed .
1. I want to ask a question since I am on daily prep and only doing condom protected vaginal sex do I really need to worry about hiv even condom fails and partner is hiv positive
2.  though I always ask my partner about her hiv status......they may lie don't know .......but do you really think I need to do that as I am on prep .
3. Also please suggest me anything extra I can do to keep myself safe from hiv .

I know questions are bit repetitive but your answers always give me ressaurance .

1.  If you were not on PrEP and were exposed to an untreated HIV infected female partner without a condom you would have a 99.9% chance of NOT being infected.  On PrEP your the likelihood of infection to substantially less than 1 in 10,000 - not something to worry about.
2.  I think asking is still good practice.
3.  You are doing thing right
