[Question #13800] Neuro symptoms

Dear dr. Hook/dr. Handsfield,
Apologies for the message, but I would, once again, appreciate your clarification. I would like to emphasize that i do Not doubt your previous responses; I am reaching out again because of the persistence and evolution of my symptoms and my difficult reconciling with them.
The current symptoms I have:
- sudden bilateral weakness in arms/legs and numbness
- difficult walking
- random muscle twitches in extremities
- eyes pain, pressure, burning
- sudden onset of eyes floaters
- tinnitus
- sternal pain and sudden palpitations
Last treponemal test was 5 months after exposure and 3 months post doxy. Along with STI panel (HIVAgAb, HCV, HBV, NG/CT, Urea/Mycoplasma, Trich) EBV/CMV show past infections. Bartonella IgM/IgG, and Quantiferon TB are negative.
When I had the rash I went to dermatologist who described it macular, and urged me to retest in 2 months because my symptoms sounded suggestive of syphilis, and doxy could have delayed antibodies.
2 weeks later I suddenly developed bilateral eye floaters , and 2 weeks after that - occular pain, and I had to go to ER where they confirmed everything was ok (slit lamp/OCT), and reffered me to neurology, second ophtalmologist said the same.
A day later i developed numbness and weakness in my extremities suddenly with tinnitus and vertigo.
Also visited a psychologist and it helped a bit with anxiety, but not with physical symptoms.
I am completely lost in this. Could it be that this is still syphilis/neurosyphilis?
Is CSF test or a retest needed?

Welcome back but I'm sorry you found it necessary.
As already discussed at length, there is no possibility you have syphilis. And your symptoms do not fit well syphilis anyway. Despite their "physical" nature, it seems likely that most if not all you describe are the result of anxiety. I will not speculate further about possible causes; you need to keep working with your own doctor(s) including your psychologist.
Believe your test results and re-read your two previous threads. Do not ask again about syphilis on this forum. Thank you.

Thank you dr. Handsfield for your answer. I am currently under the care of neurologist and i have an urgent MRI apointment in 3 days. Apparenly, I am unstable on Romberg test.
Another thing: man that I got BJ from almost 6 months ago went for RPR and full STI panel. His results will arrive on monday. (He had syphilis 13 years prior and was treated). He doesn't have any symptoms at all, and is only going for testing because I asked him to.
1. Am I 100% in clear if his RPR is negative?
2. What values are considered negative in previously treated syphilis on RPR test?
3. If his RPR come out in higher numbers, does that warrant a further syphilis investigation on myself?

I'm glad to know you're in a neurologist's care and wish you well for a favorable outcome of your evaluation including MRI results. But since you do not have syphilis or any other STI, there's nothing more this forum can offer you.

1. You're in the clear for syphilis on account of your own test results. It doesn't matter if your past partner turns out to have it; you didn't catch it. His health status should no longer be your concern.
---2. If an RPR test is positive, the strength of the positive result (titer) is determined. Any titer from the lowest of 1:1 to the highest, sometimes 1:256 or even more, indicates current or past (resolved) syphilis. There is no particular number that is always important, but in general higher titers are more likely than lower ones to mean active syphilis.
3. No matter your partner's RPR titer, it will make no difference to your situation. You may ask him and if he agrees to share the result, that's fine. But you must not insist. He has no obligation to reveal his syphilis status to you, since you were not infected.

Dr. Handsfield,
I have an update of the situation.
Partner called me, and as expected, tested positive on TPHA, but negative on RPR. ID national clinic told him that he is not an active carrier of syphilis because of the values of both tests. He also tested negative on HIV Ag/Ab, HCV, HBsAg, NG,CT.
OTOH, I've been told that i have some lesions in spine, and billateral optic nerve abnormality trough VEP.
Can I absolutely close this chapter, and move on to further testing for neurologic/other causes?
If so, I thank You for dealing with my health issues, and anxiety again. Your answers really help people navigating trough stressful times.

Yes, you can "absolutely close this chapter". Discuss with your doctor about possibilities other than syphilis and whatever diagnostic tests they may recommend.
