I 50m have hpv, gw outbreak over past year, hadn’t popped up in years before this. Wife 43f has never tested positive for anything. She did get vaccinated during our relationship. She’s nervous, what is reality of the situation/risk to her.

Welcome to our forum. Thank you for your questions. I’ll be glad to provide some information. You do not provide any information regarding sexual exposures to persons other than your wife.
Genital warts are relatively uncommon amongst persons over the age of 50. Are you confident that the lesion you mentioned is a wart? The are many other common, age-related non-STI genital lesions which are commonly mistaken for warts. How long has the lesion been present? In addition, has it been treated? Finally, have you ever had warts in the past?
Most recurrent HPV infections in persons over the ages of 40 or 50 represent reactivation of long-standing, quietescent infections rather than new infections. There are several reasons for this. Over 80% of sexually active persons acquire HPV infections within their first few years of sexual activity, and after that the occurrence of new infections drops off substantially. Thus, it is likely that you may have had HPV in the past and the lesion you described represents a recurrence. On the other hand if the lesion you describe is a genital wart and you have had recent exposure to persons other than your wife it is possible that this is a new infection. Irrespective of whether this is a new or older infection, depending on the frequency with which you and your wife have intercourse, it is more likely than not that she has already been exposed. This fact, combined with the fact that she was previously vaccinated against HPV and they vaccine related protection is long-standing makes it unlikely that your current infections represent a danger to her or that there is any reason to abstain from sexual contact at this time.

No other sexual contact besides my wife for nearly 10 years of our relationship. Yes, had genital warts a couple previous times way in the past and early in this relationship 9-ish years ago. This latest event first appeared a year ago, a small area with a few flat pink lesions, dermatologist treated with cryo for several months and though it was gone but same area reappeared in september, treated with aldara through december, again derm said looks ok but have not resumed sexual activity.

Thanks for the additional information. Your recent lesions are most probably a recurrence of your prior infection. I certainly see no reason to change your sexual practices. No reason not to resume your sexual activity. EWH---
