Hi,
I am looking for a bit of guidance please as I’m struggling to function with worry.

Very brief background that I have a bit of an issue with alcohol and anxiety, particularly relating to contracting STDs. I have managed to significantly cut back on alcohol however a recent episode is cause me significant distress.

On February the 8th I was out with a few friends and had a few too many drinks, I remember most details but have a bit of a haze going home at about 8.30 pm, sounds ridiculous I know.  

- Fast Forward 48 hours and I began experiencing mild pain and discomfort on my right testicle, not necessarily painful to touch, nor did what I believe is the epididymis necessarily any more sensitive than it would usually be, this continued for a few days, during which time I paid close attention for any discharge, which wasn’t present and no discomfort on urination.
- by the end of the week(Friday) the sensation was worsening my anxiety if that makes sense, so I perhaps foolishly self prescribed doxyxycline, of which I have taken 7 100mg doses to date, in an effort to try and move on.
-today (middle of day) I noticed a very small amount of white staining to my underwear which has sent my mind wild. I would have been checking carefully before this and was certainly nothing up to now.

Can you provide me with any guidance please, as I am close to breaking point.

Welcome to the forum. Thanks for your question.

However, I don't think I understand. You don't mention any recent sexual activity. Do you mean to imply you might have had a sexual experience during your alcoholic "haze"? I'm certainly not an expert in alcohol use (or overuse) or its effects. But my understanding is that unless you live only a very short distance from where you were drinking, and if you were able to get yourself home, there is little if any possibility of something as major as a sexual experience that you do not remember. If you have not been sexually active in the past few weeks, there is no realistic change you have sexually acquired epididymitis. In any case, STDs are not the only cause of epididymitis. How old are you? Over age 35, most epididymitis is not due to STD.

Second, testicular discomfort alone is rarely if ever a symptoms of epididymis. Absence of urethral discharge also argues against an STD as the cus of the problem. It was a mistake to take doxycycline:  its most significant effect might be to make diagnosis of any infection more difficult than otherwise. Such discomfort occurs from time to time in entirely healthy men.

As for scant discharge after a few days of doxycycline, it might be a minor yeast infection (a common side effect of broad spectrum antibiotics like doxy). It also may be possible you're just too alert and observing a bit of normal mucus that otherwise would not be bothersome and perhaps not even noticed.

Consider seeing a doctor, especially if your testicular discomfort continues. I'll be happy to comment further if you'd like to tell me more about your sexual lifestyle and what sorts of sexual experience(s) you have had in the last several weeks. But from what I know so far, I very much doubt you have any STD.

Thank you for the response.

I am a 37 year old male with no recent sexual history.
My worry is that during the brief period I can’t recal going home, I somehow was involved in a sexual encounter leading to the onset of the “symptoms”. As crazy as it might sound, the anxiety inducing effects of alcohol are quite profound and the mind will fill in blanks wherever there is any space for doubt.

To address the specific worry,, would a chlamydia infection show up as Epididymitis in 48 hours? 
I am discounting the discharge as a normal secretion as I have seen nothing since.

Hi doctor, 

As a follow up I visited a GP who didn’t even examine me, and saw the issue entirely as an alcohol related issue, he said no chance epidymitis would develop as a result of an infection 2 days prior.

I am attending counselling and want to move on, I wonder could you just give an opinion on that point, would it be typical for Epididymitis to appear after two days from a new infection.

Sorry for this delayed reply. As I said in my initial comments, I can't imagine you had a sexual exposure you didn't remember. Had you come to my office in person, I imagine my response would be the same as your GP's. And no, the only sexually transmitted forms of epididymitis cannot occur after only two days. A week or two would be expected.---
