Hello Dr Handsfield I come to you freaking out about possibly catching Hiv I am having every single textbook symptom pretty much. I am a bisexual male who had two sexual encounters on back to back days with trans(born males) Before these two events I had been tested for everything and everything besides genital hsv1 was negative. On both of these encounters I engaged in really deep french kissing I am talking 10-15 minutes .I have a deep open hole and bad oral hygiene, I tasted blood on both encounters, not sure if mines or other persons. On both encounters I also received unprotected Oral, I did not perform oral. Both lasted about 10-15 minutes no ejaculation. And on both encounters I engaged in protected anal on both, with ejaculation only on the second encounterOn the first encounter I am freaking out because mid way through the anal(lasted 3-5 minutes) the bottom literally shit all over me basically. He felt horrible, but It freaked me out, i had stool all over the condom literally, and maybe some blood they tried to clean it with a alcohol wipe but ai told then I would go to bathroom and take care of it, I am freaked out the alcohol could have possible made it go through the condom. On the second encounter, I am freaking out because the bottom kept lubing her butt(fingering) but also putting lube quickly and jerking me off. I am super terrified because at the moment I didn’t think about it. But basically the bottom, 90% sure put feces on my penis and my penis opening, after the sex I had a bit of feves and blood on the condom.

The second enounter freaks me out so much , because that person is a paid worker, and on their x(twitter) they have a few videos of engaing as a bottom 3,4 videos of her having unprotected intercourse,. I am so scared , and feel I am super high risk messing with some one known to engage in a lot of unprotected msm sex. Now fast forward two weeks later , I am literally having all the typical ars symptoms. I believe I have had some slight fevers, I have had a pretty rough sore throat which lasted 3-5 days, it may have had some plhegm on it. A dry cough which lasted about 3-5 days . Alot of muscle /body aches which I neverrrr get when I am sick. I even had the worst charlie horse of my life in these two weeks I had the ars symptoms. I had a few randm cuts on my lips, and on day 30 I had night sweats it only lasted a day. But that was 100% night sweats. Also have had diarrhea even my stomach was making loud noises that was not hunger, I felt twitching around abdomen liver for 3-5 days as well, and I have had red type of spots, pimple looking rash on my upper arms, shoulders, and my back, and a few around chest neck area. And another super random thing that has never evr happened to me I had extremely dry elbows, and knees abit , and nausea. The elbow ones were so bad I had to put lotion every hour. Never in my life has this happend. These all were between the 2-4 week, 4 1/2 week mark. And literally all completely almost disappeared now. The timing is making me think my life is over, and I was infected. And during the event I was having a genital hsv1 outbreak so I may have had open sores. I think my life is over, what is the chances I contracted hiv

Welcome to our forum. Thank you for your confidence in our service. I know how concerning the sorts of symptoms you experienced with muscle aches, cough, rash can be however, I hope that my response will be somewhat reassuring to you. The exposures you described are not as high risk as you might think. The fact that there was stool on the condom and probably on your skin following your second exposure is distressing, but is not particularly high risk compared to the risk associated with unprotected exposures.
It is not totally clear to me when you experienced the onset of your flu like symptoms. The HIVARS most often has its onset between 10 and 21 days following an exposure. Thus your symptoms appeared to have had their onset a bit late to be the ARS.Further, I should point out that in scientific studies when persons with typical ARS symptoms are tested less than one percent turn out to HIV. The remainder have far more, common, non-STI viral illnesses, such as influenza, Covid, or the community acquired non-viral STI‘s that most people get from time to time.
At this point, the single best thing you can do is get a test for HIV. I am confident that the results will be reassuring. Now, more than four weeks following the exposures you describe, your tests will provide definitive information as to whether or not your symptoms were due to the to the HIVARS ( persons with the ARS, always have positive tests, which stay positive after their onset) and will provide results related to your risk for HIV overall that are more than 98% conclusive.
I hope that this information is reassuring. At the same time I understand your concerns and urge you to get a fourth generation test for HIV as soon as possible so that you will be able to move forward. EWH.

Hello doc thanks for the quick response. Maybe I didn’t write correctly but my symptoms started appearing about 16 days after the encounters, so right around where most places including you say it occcurs, and that is what I can not stop thinking about, the timing is just way to o coincidental , I literally can’t sleep and have been drinking so much, to try to forget about it. I will have to get tested but am so terrified to test. Some of the symptoms I literally never get no matter how sick I have gotten. What scares me the most is that the second encounter the person literally has alot of videos bottoming without condoms, which clearly states they do not care about their health as much as others. I know that the gay community is doing that more than ever now since prep and doxy prep exist. I am just praying that this individual is negative, and is on prep, and that is the only reason they have unprotected so much with random hook ups. 1- you said you would not consider this a high risk event is that correct? Can you elaborate more, Stool and possible blood on condom wouldn’t make this more risky?
2-how about the deep french kissing, since it is male on male
3- how about the receiving unprotected oral during an active outbreak?
4- i kniw its difficult to say an exact number, but what would be a gestimate percentage you think I am negative, let’s say they were both hiv positive and recently infected, i know it sound hypothetical, but given the extremely risky behavior they have it is very possible
5- how about the hand genital with bodily fluids on their hand to my penis
6- I was thinking of buying a hiv at home kit, I am 7 weeks into the encounters, would you recommend it?
7- if it comes out negative would I be in the clear or are they not as good as actual blood tests?

Thanks for the additional information on timing. Indeed, it is during a time when the ARS typically occurs. That said my assessment is in no way changed. As I have already said in situations, such as yours over 99% of people have something other than HIV. Regarding your somewhat redundant follow up questions. :
1. Neither stool nor blood on the outside of a condom or on your skin poses a risk for acquisition of HIV.
2. French kissing with a person of who has HIV, irrespective of their gender does not pose a risk for HIV
3. No one has ever been shown to acquire HIV from receiving oral sex. You are not going to be the first.
4. There is a less than 1% chance that you acquired HIV from the encounters you described.
5. HIV is not transmitted by hand genital conduct. Mutual masturbation in which persons get each other’s general secretions on each other is considered no risk for HIV.
6. Why bother with the HIV test? You would have to send that off to have the test processed. You can get your results more quickly by going to a clinic or laboratory and having the test taken.
7. This too is repetitive. As I said above, if you have a negative test at this time that would prove that your symptoms were not due to HIV and would provide a conclusive answer that you had not acquired HIV from the encounters you describe. Tests for HIV are conclusive anytime more than six weeks after a possible exposure.
My sense is that you have been searching the Internet and may have been misled by some of the things you saw there. Please don’t waste time and let your anxiety continue to trouble you when you can resolve these questions quickly and decisively with an HIV test. EWH.

Hello Dr Hook I have received my results , from tests I did a few days ago. All stds including Hiv came out negative, it was a 4th generation I saw it said not reactive, I am still a bit scared since the test was done at 5 weeks and 5 days(on day 40) I am seeing some places online say that it can take up to 3 months?? Did I end up testing too soon . Also you guys seem to think 6 weeks is good, but I tested two days before the 6 weeks, should I have waited a few more days or weeks?

Do I need to retest? Or am I 100% negative from these encounters? Also one final question doc. I am some one who always uses condoms, and I only emgage in these msm things maybe 5 times a year, I am so terrified of hiv I avoid it alot. I have read up about prep on demand, 1 ) would it be a good route for some one whk only engages in this lets say once every two, three months?
2) how effective is prep on demand without condom use( I will always use condoms regardless) just wondering i dont see much info about it online

Final responses. You test results conclusively prove that your symptoms were not due to the HIVARS further the fact that you have negative tests 40 days after your encounters is also proof that you were not infected with HIV. I see no reason for further testing and urge you to move on without concern.
You may be a candidate for PEP on demand. This however, is something you should discuss with your own regular Doctor Who can discuss the pros and cons of this approach with you as well as also arrange for a follow up testing.
