Hello Drs. I have been thinking a lot and I’m worried about something that I just learned about. Hpv. In total I have had 5 protected oral encounters outside of my marriage. Btw we have only ever been with each other for 11 years. I had 4 encounters 2+ years ago in a span of a a few months. And then in January I had that encounter that we spoke about. They were all the same. Protected oral and that was it. I never knew that hpv was even a thing up until a couple weeks ago when my wife’s doctor advised her to go get a Pap smear done. Since she hasn’t done one ever in her life. She is going to be 26 in a few months and I am 28. Reason why she’s been prolonging it was because when she was around 8-9 years old she was sexually assaulted by a family member and never went to get checked if the individual gave her anything and her family never knew about it so they also didn’t take the proper precautions to have her checked. To her knowledge she wasn’t penetrated with his penis it was just his fingers as well as he had him touch his penis as well. Besides that she has never had any other sexual contact. We are both vaccinated but I don’t think we have the one that carries the 9 strains of protection since we got the vaccine at a young age. I got my vaccine at the age of 13 in 2011 and my wife got hers at the age of 11 in 2012. I only got 2 doses of it and she received 3 doses. I did have a relationship before my wife but we only ever kissed and french kissed. That’s it. With all that info I have some questions I hope you can help me with. I also want to point out that the 5 encounters I have had outside my marriage are the EXACT same. A prostitute. That I picked up and gave protected oral.

1. What is the chance that I got hpv and gave it to my wife? 
2. I haven’t had sex with my wife since my last encounter because of fear of giving her something. Last time we had sex was last December. Since I had an encounter in January with a prostitute do u think I should wait to continue sex with my wife or should I wait for the Pap smear she has in April? 
3. With the vaccine we received back in 2011-2012, is that going to lessen the chance that if a hpv strain were to take hold it would clear it faster. 
4. Since it’s been 2+ years since my first 4 encounters would it likely that her Pap smear won’t show anything abnormal and hpv would be negative if I got a strain from one of the 4 women? 
5. I’ve been reading a lot on this forum and I have leaned a lot about hpv especially when it comes to oral. But does having 5 different oral encounters higher my chance of getting hpv.

6 I have put most of my worries away when it comes to every other std/hiv. I tested negative for gonorrhoea chlamydia. I also tested negative for HIV at 4 weeks since my last encounter. I also took 7 days of doxy as well as 1g of azithro. On top of that I am on a course of 2 months of doxycycline for my acne. Other than hpv is there anything else I should be worried about? 

I have been working on my anxiety levels. I am taking ur advice and trying to set up an appointment with a psychologist so I can get over this guilt. Thank you for ur help.

7. Just for my wife sake, what are the chances that she could have go hpv from her assault and it comes back up now?

8. I haven’t had sex with my wife as I said can I have sex with my wife and know that her April test won’t cause a positive hpv result.

We ask clients to limit their questions to the specified work limit.  You have exceeded that and your questions suggest that you are quite anxious about this.  I think you are worrying more than you need to.  Oral infections are less common and more poorly transmitted than genital infections.  In addition, you and your partner's vaccination will protect against HPV types most commonly associated with oral infections.   Brief responses below:

1.  Low.  See my introductory comments above.  In addition, most HPV infections are self-limited, clearing without therapy in 6-24 months.  It is unlikely that you acquired HPV and if you did from your exposures over 2 years ago, those infections have likely resolved.
2.  I think that you are over reacting.  If you had/have HPV she has been exposed, if you do not, then there's nothing to worry about.  
3.  Yes, your prior vaccination greatly reduces your risk of infection.
4.  PSP smears are recommended for all women.  Her likelyhood of having HPV is relatively low.
5.  Each encounter increases risk somewhat but your overall risk remains low.
6.  I would not worry about other STIs including HIV even without testing.  Your tests reinforce that you have little to worry about.
7.  Some people have HPV without a good explanation however HPV is not meaningfully transmitted through hand to genital contact.
8.  I see no reason for you to abstain from sex, as I mentioned above.

I'm please to hear that you are addressing your anxiety.  Please continue to do so.

Thank you Dr hook. Just a few follow up questions. Also I apologise for asking too many questions. 
1. Is there a percentage you would be able to give me based on my encounters. Im sure its just ur choice of words but ive seen u use “close to zero” is mine more of a risk? 
2. Did the condom do anything to stop transmission? 
3. From your experience which strains are found more commonly from recipient of condom protected oral? 
4. I know you said not to worry about anything like hiv. But I’ve been having diarrhoea for 4 days lately it’s been 7 weeks since my last encounter. I tested negative at 4 weeks should I be worried? 
5. So having sex with my wife now wouldn’t make the test in April positive if I go it from the escort in January?

I also do have a question for you, Dr Hook. Is the reason why you guys don’t ever really mention HPV when you guys see individuals ask about an oral sex event because of how rarely HPV is transmitted with that type of act or is there a reason why you don’t really mention it to individuals that they’re at risk of acquiring and transmitting HPV?

You're clearly anxious.  I'll do my best but need to caution you that data for HPV are limited because the the disease are so very prevalent.  On to your questions:
1.  We base our advice on sound scientific data.  Those data are limited when it comes to HPV.  I cannot provide you a number.  Based on your history, the likelyhood that you have HPV is low- less than 5%.  That's just a guess since there are NO data to inform my reply.
2.  Condoms certainly reduce the risk for genital HPV and logically that reduction should be lower for oral sex.
3.  HPV 16 which is in the vaccine you received, is the most common form of oral HPV infection.
4.  No- your diarrhea is due to something other than an STI.
5.  As I said above, I see little reason to abstain from sex with your wife.

Oral HPV related to oral sex is something that our clients worry a great deal about but is  relatively rare and when it is present, determining how long it has been present and the natural history is difficult

1.Okay so all in all with everything from type of exposure to condom use to vaccine to my first 4 encounters being 2+ years ago and most recent one was a couple months ago. Would u say my chance of getting hpv is very low? Sorry if this is a repeat I just wanna see what are my chances. 
2. Just for my own understanding, because I’m vaccinated for hpv 16 (which is the most common strain in the oral cavity.) am I understanding it correctly that I could still get it? And also if I did still get it the vaccine would help eliminate it from my body faster in my body than if I was vaccinated? 
Is there anything else u think would benefit me in finding out more things about hpv? As I said my wife’s appointment is in April and as u guessed it my anxiety is high because if she is positive for hpv I know she will prob accuse me of cheating and I wouldn’t know what to do. Any advice would help greatly Dr. Hook thank u for ur patience.

Final responses:
1.  Yes, I would say  that your chances are very low however I cannot put a number on it.  
2.  Vaccination reduces risk for HPV 16 about 97% in scientific studies.  If you got it, the likelihood is that it would be eliminated by your immune system over the next 6 months to two years.

Even some people who have never had penetrative sex have been found to have HPV.  Your wife's PAP smears is likely to be normal.  I suggest you wait for the results without concern.  EWH
