Doctor, I have two questions to ask you. The first one is that I had some drinks with friends tonight and then found a technician online. At first, we did oral sex with a condom on. It took a long time and I couldn't ejaculate. Then I changed to a new condom and had sex. Afterward, I didn't check the condom carefully. Later, when I was taking a bath, I casually mentioned to the technician that there are many cases of HIV infection in this line of work. She said yes, and several of her friends have it, and one even lives with her. This made me break out in a cold sweat. Then she said something even more terrifying: some technicians know they are infected but still serve customers, and they prick each condom with a needle. I recalled that this technician was rather thin and her skin was wrinkled. So, in these circumstances, is there a risk of infection? Do I need to take a preventive medication? I'm quite worried and would like you to analyze the situation for me. The second question is that a few days ago, I masturbated and broke the skin of my foreskin. After a few days, it seemed to have healed because it didn't hurt when it got wet. Then I went to a spa. The technician didn't wash her hands and directly touched me without using any lotion. She also licked my chest and bit my nipple with her teeth. Afterward, my nipple was a bit sore, but I didn't see any blood. I don't know if my skin was broken. That's the situation. If, in the worst-case scenario, the technician's hands were cut and bleeding, and my nipple was bitten and bled, is there a risk of HIV or syphilis infection? Supplementary content: Why would they use needles to puncture the condom? Some technicians, after learning they are infected with HIV, have a desire to retaliate against society. Now I'm very worried. Please analyze whether I should take the blocking medicine immediately and whether I need to go for a test later.

Welcome to our Forum.  Thanks for your questions. I'll be glad to comment.  

Regarding the 1rst encounter you describe, the risk is unknown.  Most commercial sex workers do not have HIV and even when they do, condoms are highly protective.  Even in the statistically unlikely situation that your partner had HIV and had put a pinhole in the condom, the likelihood of infection from a single encounter is less than 1 infection in over 2000 sex acts.  In other words, IF she had HIV and IF she had punctured the condom, there is still a greater than 99.95% chance that you would NOT become infected.  If has been more than 3 days since your exposure, the only way to know if you were infected is to test.  An HIV RNA PCR test can conclusively rule out infection any time more than 11 days after your encounter.  If the PCR test is not available, a standard, 4th generation test for HIV would be conclusive any time more than 6 weeks (42 days) after infection.  

For the second exposure, there is no meaningful risk for HIV or syphilis from receipt of masturbation, from having your chest licked, or from having your nipple bitten.  I would not worry about that encounter.

At this time, the best thing you can do is test.  How long has it been since the first encounter?  EWH
