Hey Dr how are you
I just am looking for some reassurance and some answers I’m in Australia and about 3 months ago I had protected sex with 2 guys about a day apart from each other, with 1st guy was mutual oral and I was also the top protected but he did rub it on his anus a bit without condom but didn’t go fully in without condom, 2nd guy was similar but he just gave me oral and I would say no major contact without condom I was top again, both times oral was without condom and I got gonnorrhea and got that sorted and TOC was clear a month after antibiotics but still have sore testiculs also from this I have had immense anxiety about hiv from this I have been tested with a 4th gen lab at 9&38 days also have had 2 atomo rapid test at 38&71 days all negative should I still be worrying about this.
The atomo rapid tests for hiv1/2 also done by the sexual health nurse both times

Sorry I forgot to say I had gonorrhea in my throat and penis and I did perform oral on the 2nd guys anus

Responding to the title you chose for your question, before reading anything else: With a condom protected sexual exposure, in all likelihood there is no need for testing for HIV or any other STI.
Now have read the rest: First, Australia is the most advanced country in the world in regard to HIV/STI science and prevention. Your nationally funded sexual health clinics are absolutely superb; your local SHC -- if that's where you intereaced with the sexual health nurse -- is an excellent resource. Second, partly for that reason, if the nurse you was at one of those SHCs, I'm confident you can rely absolutely on their evaluation and advice. Third, you describe sexual exposures that don't seem to be very high risk -- condoms work! In addition, HIV is not transmitted oral to penis: there has never been a proven case of HIV transmitted that way. Fourth, your negative HIV test results are entirely reliable. It is completely certain you don't have HIV.
Therefore, in response to your concluding question "should I still be worrying about this?" No, absolutely not!

I but i got gonorrhea from one of them and didn’t have unprotected sex also worried that I have tested too early and hiv may have been missed

With apology, I'm embarrassed that I read your question too quickly and missed that you got gonorrhea. Most likely it was from the oral exposure: gonorrhea is the main STD commonly transmitted oral to genital. However, despite that fact, it remains true that HIV infection is very rarely acquired by oral sex, if it occurs at all. And the HIV test results you report are conclusive; those tests were not too early to diagnose HIV.---

That your HIV test results are conclusive means there is no possibility the result will change until and unless you have a new HIV exposure.

Testicular soreness without swelling or tenderness is not a symptom of HIV or any other STD. Genitally focused anxiety probably is the best bet.
