[Question #13998] HIV csw

Hi, very stupid man from London. White, 46 and circumcised, heterosexual. After work a few of us got drunk and ending up at a strip club. Anyway I then ended up at a brothel on my own. No recollection of how I got there but i remember picking a girl from a line up and remember her putting a condom on my non erect penis. Then all I can remember is trying to penetrate vaginally but don’t think I did as I think I gave up trying after 2 or 3 goes. I sucked her breasts and we kissed but that’s all I can remember. I don’t remember not having a condom on or it breaking but can’t be sure. Never ever done this before and the only time I have done a hiv test was 6/7 years ago when I started a new relationship which was a negative rna pcr test. The exposure happened 4 days ago. I am planning to test for chlamydia etc next week but I would like to know my odds on hiv and whether I should not have sex with my girlfriend. Should I even test? Thank you

Welcome to the Forum. Thanks for your questions. I'll be glad to comment. This was a low risk encounter. Most commercial sex workers do not have STIs, including HIV and most single encounters with infected partners do not result in transmission of infection. For HIV a singe unprotected exposure to a woman with untreated HIV results in infection on average less than once in over 2000 exposure. Further, STI rates in brothels tend to be low as infection of clients is bad for business.
I suspect that if you had managed to penetrate her, you would have remembered it.
I would have little concern about STI risk. I understand that you may chose to test for the reassurance that a negative test can provide. A urine test for gonorrhea and chlamydia ill be conclusive any time more than 4 days after your encounter. Testing for HIV is less important in my opinion but if you wish to test, an HIV RNA PCR test will be conclusive at any time more than 11 days after your encounter.
Personally, I would probably not bother to test. Likewise, because it is so unlikely that you acquired an STI, I see little need to abstain from unprotected sex with your regular partner. EWH

Thank you for your reassurance. I don’t remember much but I do remember me trying to penetrate her with a non erect penis and I’m sure the condom which she provided and put on was on however my fear is HIV. Due to not being erect and if the condom fell off and all it was was frontage (my penis touching around her vagina) would you still be concerned? To add this happened in London and the girl was from South America. Trying to find out statistics in London and it looks like less than 1% of sex workers have hiv. Probably my ocd kicking in as I have never done this before

I have learnt a lot from this forum. Just noticed I could have requested pep. Would you have prescribed tgis for my exposure? Thanks

You are correct that HIV rates among commercial sex workers in London are low. Frottage is a no risk activity. Your risk if HIV from this encounter is close to zero. I would have no concerns and certainly see no need for PEP. EWH---

Thank you. Hopefully my anxiety will calm down now. Last question. I plan to get tested to be on the safe side for chlamydia and gonorrhoea as I can do this straight away and a higher chance of infection. If those tests are negative I was going to leave it at that. Would you agree that should be enough? I was only going to test HIV if either the others are positive as my risk would increases from near zero to a few percent? Just to add this is my first and only exposure outside my current long term relationship. Obviously there is guilt and anxiety and wishing to keep my partner safe. Thanks

Final response. You catch fish that’ll be good. A urine test for gonorrhea and chlamydia anytime more than 3 to 4 days following it encounter will provide inclusive results. In your case, I am confident, that should you test the results will be negative. Further, in the circumstance that a test would be positive (again. I believe this isunlikely) your risk for HIV would only increase minimally.
As you know, we provide up the three responses to each client’s questions. This is my third response and therefore this thread will be closed following this response. Please don’t worry. EWH.

Conclusive when testing is performed 3-4 days after an exposure.
