UNCIRCUMSCISED male from Mauritius.  I had an encounter with a female Thai CSW who is in Mauritius for sex work.  I have genital herpes and at the time of the CSW encounter I had sores which had healed and scabbed on the mid shaft of my penis.No open sores.

I performed cunnilingus. Unprotected BJ.  protected vaginal sex for around 1 to 2 min max.  Condom broke inside and tip of my penis was exposed.  The scabs on my shaft were still covered by the condom.  She performed unprotected BJ on me for around 2 min. I plan to get a gonorrhea and Chlamydia PCR urine test by next week. HIV and Syphillis test at 6 weeks.  CSW said she tested negative HIV 3 months ago in Thailand. I want to have unprotected sex with my partner if tests negative next week.  Pls Advise.

Welcome to our Forum.  Thanks for your questions.  While it is not your question, I presume you are aware that you may have transmitted your herpes to your partner if she had not already been infected if your condom slipped down during sex even if the condom had not broken.

Your plans to get tested for gonorrhea and chlamydia are good and will provide conclusive results.  Your risk for syphilis or HIV is very low.  If your gonorrhea and chlamydial tests are negative there is very little risk of transmitting an STI to your regular partner with unprotected intercourse even before your follow up syphilis and HIV test.  EWH

Thank you Doc. I thought once herpes sores have scabbed they are no longer contagious. That’s what I was told. I’m sorry.  I truly am. Once long ago I was diagnosed with herpes this was what the nurse had told me here.  I never had any partner outside my regular partner from whom I got it. This is the first time I saw a csw in my life. 

If my hiv and syphillis tests also are negative after 6 weeks, should be worried about anything? I’m just scared.

I’m sorry to tell you that you were misinformed previously about the contagiousness of herpes lesions.  Please avoid sex with uninflected partners until lesions are entirely resolved.

As I said above your risk for HIV and syphilis is very low.  A small fraction of 1% for HIV and less than 1% for syphilis at this time.  With negative tests at six weeks there will be virtually no risk.  EWH

Thank you Doctor. I’m really sorry.  I didn’t genuinely know about herpes. I was misinformed. I shall never engage into sex if I have lesions. I just feel really bad. I never wanted to infect anyone ever.  This is the only time I have been with another woman except my regular partner.  My regular partner has herpes and I got it from her.  Sexual health is still very much a taboo in my country unfortunately. 

I’m just very very very worried about HIV.  Thankfully the HIV duo combo test is available in a private clinic which I will do at 6 weeks. I’m hoping that will be the end of this mishap and I can put it all behind me. The wait is very much a worry. I just want to be in the clear.  I feel really bad. Thank you for your help.

As I said, your risk for HIV is very, very low. FYI, giving you anxiety, if you want reassurance, at four weeks following your encounter a fourth generation test for HIV will detect well over 98% of recent infections although it is not conclusive until after six weeks. The four week information might be comforting. Alternatively, if you wish to accept the additional expense of the test, anytime more than 11 days after you were encounter an HIV RNA PCR  test will provide conclusive results as well.
