[Question #14138] Worried, follow up on #12762

Couple of months since the vaginal stinging and itching of my wife, I developed symptoms of perineal pain and sensation of warmth at the beginning of urination (when I relaxed the external urethral sphincter). Pain onset was all of the sudden and at the time had brown bits on my semen which persisisted for 2-3 months. I presumed this was hemospermia which I also had (with obvious blood back then) 4 years before this. I was diagnosed with chronic prostatitis in the past a couple of times 15 and 8-10 years before that, might had perineal pain or similar dysuria in the past which I disregarded. I might have had gardnerella vaginalis in semen cultures in the past but past partners had it (I know it's part of normal vaginal flora). Did repeat NAAT, this time through a urethral swab (quite invasive, all the way down), which came back positive for gardnerella vaginalis but negative for everything else, including trich, mgen,ugen, gonococcus. Semen culture straight after the naat had gross blood in it but came back negative as urine culture as well. Wife never had any symptoms again. Occasional urethral stinging (at the level of the base of the penis that can migrate to the shaft but not the tip) more like a sensation of warmth was on and off. Last couple of months, same urethral stinging and perineal pain, which is not present when I'm relaxed or in the morning. Urine dips negative (might find occasional leucocytes but after the 2min window the test suggests). Spoken to urologist friend (I'm a doctor) who are quite confident that this is cpps and said no further tests needed. Since symptoms are back, and dysuria can be perceived as quite significant) shame is stopping me from getting further tests. I'm worried for a missed bacterial sti and especially missed gonococcus that I get by my theoretically asymptomatic wife by re-infection. Should I stop worrying or investigate further with repeat NAATs and maybe prostatic massage meares-stamey cultures and naats?
