[Question #2071] CBC abnormalities

Follow up with Doctor HHH on a previous question. I did get the results of the gen4 test and it was negative. Although, I also did a CBC and several abnormalities were shown.
4/20 was the first CBC followed by another one on 4/28. Here were the big differences;
WBC 4.3 to 3.6
Absolute Lymphs 1.2 to .6
There were also an abnormalities with Protein and Ketones. Also low was the Bun/Creatinine Ratio and cloride/Serum level. Any insight would be much appreciated. Tests done to date:
Day 8 HIV RNA TMA
Day 15 HIV RNA TMA
Day 23 Generation 4

Matt, today I will be answering your question rather than Dr. Handsfield. FYI, we have worked together for over 30 years and while the some of the verbiage and facts of our replies sometimes vary, the fiacts and bottom line do not. I have also reviewed your earlier interaction with Dr. Handsfield and agree with all that he has told you already. My sense is that you are overly anxious and paying attention to variables which are of no consequence to you in terms of risk from the exposure that you are concerned about. Your WBC is on the low side but not dangerously so and likely is normal. Same for your lymphocyte count. The important FACT is that you now have 2 negative HIV RNA tests and a negative 4th generation test which, in combination incontravertably PROVE that you did not get HIV from your exposure of concern. It is time for you to accept this and move forward. I hope that my comments will help you to do this. EWH---

I thank you for the time you both took to read and consult on my concerns. I am ready move on from this and make some big changes in my life. Can I just ask a few questions from today's test and then I will accept things as they are.
1) is the drop, in one week time, of the absolute lymphocytes and WBC a concern? Could this be what the start of the HIV ars? What would cause this drop?
2) is the timing of the tests at all worrisome as just a slight bit early for the RNA and maybe a few days early for the Gen4?
3) I am HSV-2 positive, does that change any of your opinions? Could HSV-2 have any effect on the blood numbers?

1. the change in your WBC and lymphocyte counts are unlikely to represent a trend but rather day-to-day fluctuation. this is normal and not something to worry about. As I said above, you do not have HIV. Get over this- you have proved that you did not get HIV.
2.As Dr. Handsfield has already told you, the combination of your results has provided a definitive result. You did not get HIV!
3. This too is a redundant question that Dr. Handsfield has already answered. While your herpes may impact your risk, it does not change the FACT that you did not get HIV from your encounter of concern. EWH

Hello Doctors. A couple weeks have gone by and I have excepted the negative HIV results with your assurance and the anxiety levels have come way down. Thank you. I believe a lot of the "symptoms" were do to the anxiety and possibly of the recent HSV2 diagnosis. Knowing I have herpes I believe I am going through my first out break. I do not have any kind of breakouts on my genitals yet, but I am curious if the several small red pimples on my inner thighs and various ones down my legs and feet would be do to herpes. They appeared yesterday. They are small and do not itch at this point. Thanks for the help

Thank you for your follow-up. I'm pleased to hear that your anxiety has declined and hope that this will continue. It should. You do not provide much information about your HSV diagnosis but from the sound of things it was made on a blood test. If this was the case, I trust that it was not an IgM test or that if it was an IgG test, that the index value was greater than 3.5. If that was the case, indeed you may not have HSV at all. The pimples/red spots that you describe sound widespread which would not be typical of herpes at all. What you describe really sounds much like folliculitis which is common n the location you describe.
