[Question #2231] Protected oral sex and possible stds

Hi doctor, I'm following on from my message 2 weeks ago. 7 weeks ago I had a regrettable drunken encounter with a CSW. I had protected oral sex that lasted 15 mins or so. 2 days after the encounter I developed hives on my feet. The itchiness of the hives lasted a few hours then hives developed into a spot which lasted maybe a week or so. This developed in both feet. Then a bunch of hives developed on my lower back/top of bum. These lasted maybe a week. When I had these hives I visited an std specialist and he completed a full range of tests. This was 5 weeks post exposure. All my results came back negative for all the tests including blood test for herpes. I was wondered the hives could be herpes. The doc told me that the results just show that I did not have herpes before the exposure and to retest 8-10 weeks after exposure.  He did say though that when he seen the hives they didn't resemble herpes. He gave me an antihistamine and I have not gotten any hives since.

My right hand has since developed a tingling/burning sensation last week and upon closely examining my hand over the last week I can see patches of my hand changing colour and developing a red round border,  it's still vague but I can see it getting worse everyday. I was worried about scabies but they area is not itchy and Ive read this is the most obvious symptom of scabies. There does seem to be areas of new burrow lines developing but I'm not experiencing any itchiness.

1) In your opinion does this sound like anything I could of picked up from my exposure or is it possible to get an std on my hand. Do you think it could be scabies?

2) do you think the hives could be related to herpes? As I only had oral sex if I got herpes would they show up on my penis first?? 

3) I'm planning on repeating my blood test for herpes on the 9 week mark. Would this be sufficent? I would plan on waiting longer but I'm presently avoiding sex with my wife in fear that I infect her?

I'm sorry you continue to worry about your exposure 7 weeks ago.  I'll go straight to your current questions:

1. No this does not sound the least bit like anything you might have acquired from the receipt of oral sex you experienced 7 weeks ago.  It certainly does not sound like scabies either.  What you describe may be recurring urticaria.  We discussed this in our prior exchange.  I would see a dermatologist about them at this time.

2.  No!  We discussed this extensively in our prior exchange.

3.  Yes, that would be a fine time to repeat your herpes test.  I am confident it will be negative.  Were I you, I would not be abstaining from sex with your wife.  There is virtually no chance that you acquired something in that encounter that you could give her.  EWH

Ok thank you doctor for your quick response. Just a follow on from your answer. What's developing on my hand does not resemble a hive. It's not raised just a different skin colour is forming on the back of my hand. I'm not sure what it is at all. At first I taught scabies but it's not at all itchy so I presume that rules out scabies. I visited my gp today and showed him and while he could see what I was worrying about he didn't think it was anything serious. (I never told him about my exposure so was worried it might be something related to an std).


Secondly if I did contract herpes from my encounter and I know you don't think it is, but in the unlikelihood that I did would a herpes lesion develop on my penis? As I only had oral sex would it be possible for herpes to develop on my back or my feet? I have not had any evidence of anything developing on my penis since the exposure. 

Sorry if these sound like stupid questions.

Thanks for the additional information.  your hand lesions are not suggestive of an STI, nor of scabies.

Herpes is acquired by direct inoculation with lesions appearing at the site of contact, not at a distant site like your feet. .  Thus given your exposure, there would be no reason to consider that HSV might be occurring on your feet.  Again, I really do not think HSV is in the picture here.  EWH

Ok thank you doctor for response. I'm aware this is my final question so could you give me a brief run down of what stis you think I could of being exposed to.  As I said my encounter was protected oral sex. I've tested negative at 5 weeks for chlamydia, gonorrhoea, blood test for syphillis, hiv and hepatitis and herpes. Should I get re tested at 9 weeks and what should I get tested for in your opinion. As I said I would wait longer but I am putting off having sex with my wife because I fear I might infect her and she is getting very suspicious as to why I will not be intimate with her.

 Also doctor is it possible to get sti symptoms of any disease on my hands? I'm aware of a syphillis rash on the palm of the hand but this does not resemble that. Is there anything else you think it could be. Thank you again for your help and for the service you provide.

I really think you are worrying inappropriately about the extramarital exposure you described.  As I said in our original exchange " I really think you are worrying more than you need to about herpes or other STIs as the cause of your hives."  Not only do I not think your hives are related to your exposure but I also do not think there is any reason at all for you to be worried about STIs.  You have been tested and found to be negative.  I have little to add to what I've already said. 

There are no STIs that would cause the rash on your hands that you have noted.  I see no reason for further STI testing of any sort.  I really think you need to address the guilt that you seem to have over this exposure.  I think you are clear to have unprotected sex with your wife without concern. 

I hope that these comments have been helpful to you.  I would urge you to put your concerns about STIs behind you and reiterate that I see no need for further STI testing of any sort at this time.  To paraphrase Albert Einstein, "One definition of insanity is to keep doing the same thing over and over and expect different results>"  Repeating your tests will not detect an infection.  Take care.  EWH
