[Question #2389] Follow up on #2233

Hi Dr Hunter,

This is a follow up from my earlier question. 

To recap i wish to inform you the followings:
1. I am a male who had unprotected oral with female csw as receiving partner and protected vaginal sex. Exposure was on 31 march 17. 
2. Latest test done was pcr dna urine for gonorrhea, chlamydia, syphilis, hsv1 and hsv2 on day 63 (9 weeks)
3. For completion i also had a 4th gen done on 12 weeks.
4. Hsv1 and hsv2 bloodtest for igg. 

Negative for all except hsv1 blood test. I already have this since childhood so i think its not an issue until now. 

What is a concerned to me is the event that transpired 3 days ago which is as follows:
1. I felt itching sensation at my penis head. Slightly up where the foreskin connects. 
2. There was two very tiny sore with clear fluid inside. I think these broke open and leaving a sore like a small popped pimple scar. 
3. The inner part of the foreskin is red at some places: 
4. Slight pain in my right pelvic area. Pulling pain and more sensation when i  apply pressure on it; and
5. I have one paintul ulcer on my lips just developed today. 

I went to see a general practitioner near my workplace. Told him the history and he examined my penis. He think it is fungal infection.  He gave me two creams to apply and seems to be working. Redness n itching  does reduces quite substantially but it still itchy

My questions are:
1. Can i take the gp diagnosis as correct or should i be concerned with other std such as herpes n ors?  I accept hiv is conclusive. 

2. Urine specimen was collected at the end of the day. Mid stream urine. Does it make the tests less reliable?

Sorry for the trouble and your advise will be appreciated.

I'm sorry to see you apparently are still having trouble moving on from your low risk sexual exposure. I would advise that in addition to my responses here, you carefully re-read all my replies in your other thread -- although glad to hear you have accepted your HIV tests as conclusive.

1. These symptoms do not fit with herpes, HPV, or any other STD. They are consistent with a superficial fungal infection. I see no reason you should not believe your doctor's diagnosis and advice.

2. Although official advice is that urine for gonorrhea/chlamydia testing ought to be the initial part of the urine stream, in fact midstream specimens are believed to give equally accurate results. Time of day makes no difference. In any case, these or any other urethral infection would not cause the symptoms you describe.

You clearly are continuing to react emotionally to a sexual decision you regret. Do your best to separate those feelings from its potential medical consequences. They are not the same. Deal with the emotional factors as best you can (perhaps with professional counseling), but you can disregard the latter. You cannot go through life attributing every twinge, ache or tingle to that single sexual event.
