Hi Doctor! I interacted with an escort yesterday. Penetrative sex did not happen but she rubbed her feet (with stockings on) pretty hard on my penis for several minutes and then gave me a hand job. My question is: If she had some cuts on her feet or her hand, and a tiny amount of blood came out (I didn't notice any), was I at any risk of getting infected with HIV? We get lots of questions about non-intercourse sexual exposures. Our consistent response is that generally there is little or no STD risk. In particular, hand-genital contact, fingering, etc are zero risk for all practical purposes. There has never been a case reported in which HIV was acquired sexually except by direct vaginal or anal sex; even oral sex is little or no risk. While I don't recall any previous questions about foot-genital contact, logically it's no different than hand-genital contact. Even if there were cuts or wounds on the feet. Further, I'll point out that escorts -- meaining expensive female sex workers who operate by appointment -- are believed to be quite safe partners. They know the score, take steps to protect themselves, get tested frequently, and in general their clients are believed to be at low risk for HIV (i.e. men like you).
So you really needn't worry. I don't recommend testing for HIV or any other STD, and if you have a regular partner, you can safely continue your usual sexual practices without putting her (or him) at risk.
