[Question #2515] Conflicting Blood Results after Blot

Dr. Handsfield:

Had unprotected Oral Sex and protected Vaginal Sex at a bachelor party. Been having irritation / warmth in scrotum and shaft as well as frequent urination ever since. stopped for about a week after 1 month and came back. Warmth comes and goes. Below are my results:

6/12 - 4 weeks after- I took a full set of tests. HSV1 - high count of 38.9 -HSV2  came in a low positive of 1.89. Doctor said it could be a false positive and to go to an internist. Although the warmth and urination had semi stopped for a couple days prior to this, upon getting the results it came back full fold.  Quest ran the results

6/16 - (5.5 weeks) post exposure  - took another test only for hsv2 which came back negative.  Enzo Labs

6/22 - (6.5 weeks) -46 days after -  I followed up this test with a Western Blot -  Negative for HSV2. Positive for HSV1. But still concerned, I read the blot may not be accurate until after 68 days. 

7/20/ - (10.5 weeks post exposure)  - HSV1 IGG positive 18.1 and HSV2 Igg low positive 2.1

These are now 2 positive IGG's and 1 negative IGG as well as a negative Blot at 46 days.

Does Quest have higher standards than other labs and do they tend to give positive results more often? Do you believe I am positive. Could the Western Blot have been a false negative.

I just sent out blood for IGG testing again at 11.5 weeks to different labs. If those come back negative am I ok?

Stress is killing me as I am married and want to protect my family.

I answer the herpes questions on this board, and thanks for posting.
Yes, it is recommended that you wait to do the western blot until 12 weeks (84 days) have passed.  So you did it about a month early. 
Quest doesn't have higher standards than other labs.  All labs can give false positive results.  The CDC recommends that all people who have in index value between 1.1 and 3.5 have confirmatory testing and you did that but too early. 
You may wish to repeat the western blot, but if you test again with IgG and it is negative, I do believe you are probably fine.  I doubt that will happen but maybe. 
I understand that this is stressful for you and your family - it's very hard, I get it.  But there is honestly nothing that will give you better answers except time.

Terri,

You doubt this will happen with the igg test because you think I am positive  or because my igg scores will probably show low positive in any igg lab  and the only way to confirm is a western blot now at 11.5 weeks?

At this time would you assume enzo labs result was a false negative. 

Is it a coincidence that only quest diagnosed the low positives?

I doubt you will be positive because your values on the IgG have been so low.
Yes, I think it is a coincidence that Quest had low positivesl

So in your opinion you feel comfortable with the 7 week blot result being negative? A local doctor said they do the blot as well in their offices. Are you familiar with other doctors doing the test other than Univ of Washington?

Forgot to mention I took an initial test from labcorp - 2 days after exposure on 5/10/17 which came in HSV1 Positive - 6.0 and HSV2 negative. Does that affect your opinion at all? Also, I believe I had mouth sores when I took mouth  for 1st positive.

No doctor does the western blot in their office.  I would bet a million dollars on that one!  They might due the immunoblot but that has even more problems than that IgG test because one cannot identify the low positives with that test that need confirmation.
Herpes normally does not present in the mouth but rather usually on the border between the lip and the face or in the nares. 
The result at 7 weeks is about 70% likely to be the results if the test is taken at 12 weeks.
