[Question #2542] Anxiety and Guilt

Hello Doctor,
I am in a 20 year monogomois relationship. 10 days ago I made a poor decision and I am full of guilt and anxiety. I attended a bachelor party at an all nude strip club and had several lap dances from a dancer. During the lap dances she pulled out my penis and performed portions of the dances while rubbing her genitals on mine . There was absolutely no vaginal or anal penetration at all.
Approx 3-4 hours earlier that day I had used some new clippers on my upper/inner thighs and sustained several cuts. I cleaned the cuts, applied antibacterial ointment and covered the cuts with a clear dressing. The friction from the dances moved the dressings around a little but I am pretty sure that that part of my thihs did not come out of my pants.
The only symptoms that I have experienced is some dull Lower back pain ...which could be attributed to the anxiety.
My question is what if any tests do you recommend for this type of incident?

You are at little or no risk of any STD from these events, regardless of your recent shaving/clipper wounds. For sure there was zero risk for any infections transmitted primarily through fluids (HIV, gonorrhea, chlamydia). In theory, there could be risk for those transmitted skin to skin (syphilis, HPV, herpes), but in practice we never see it -- that is, even the busiest STD clinics virtually never see patients with these infections following exposures like yours. As for your symptoms, there are no STDs that cause lower back pain. (And whenever someone suggests his or her own symptoms have a psychological origin, usually s/he is exactly right!)
Therefore, I do not recommend any tests at all, and would recommend that you continue your normal sexual practices with your regular partner. That's what I would do if somehow I were in your situation.

Your expert opinion and recommendations are helpful. However, this feeling of guilt coupled with fear is pretty difficult to bear.
Since your answer I have experienced some coughing (that seems to be related to a post nasal drop ( (no fever) and also some urinary urgency and a feeling of being unable to empty my bladder completely (this is comparable to symptoms I have had in the past of prostatitis)
Do any of these symptoms change your opinion?

No change my opinion. For sure cough and post nasal drip are not symptoms of any STD or HIV. Sounds like you caught a cold. Urinary urgency and related bladder symptoms in males, without discharge or painful voiding, are rarely due to STDs. They are typical for genitally focused anxiety, which is common in men concerned about a sexual experience they regret. (Indeed, many apparent prostatitis cases have the same cause; there's a big overlap between prostatitis and genital anxiety. STDs don't cause prostatitis, by the way.
Having said no testing is necessary, you might want to consider seeing a health professional for genital exam and a urine gonorrhea/chlamydia test. Not because you are at risk, but because the negative results might be more reassuring than my advice from this distance. Putting this in personal terms, if I were in your situation i definitely would not do that. But I'm not you, and perhaps the negative results would help you move along without worry. Up to you.

There's nothing more I'll be able to say based only on your exposure and these symptoms. In the meantime, stay mellow. For sure you have nothing to be worried about.

Ok Doc. Gonorrhea and chlamydia urine tests both came back negative.

Doctor I Also had a HSV 1 and 2 Elisa blood test. The test was performed two weeks after incident. HSV 2 was negative but HSV 1 was positive. However, I did have a few fever blisters and cold sores as a child. My doctor doesn't seemed concerned at all about the + HSV 1 as I am asymptinatic and a large percentage of people my age (42) test positive for that. I believe I am ready to move forward and chalk this experience up as a lesson learned.

Your doctor is exactly right about your HSV1 test, on all counts. And you could not have acquired HSV (either type 1 or 2) from the exposure described.
