[Question #2650] Escort / Semen incident

Hello Doctors,

I am a happily married man in UK but in a drunken moment of madness last week visited an escort. No sex took place – she masterbated herself and I masterbated myself. So far so good. However, when I got dressed I noticed a small amount of semen (thumb nail size) on my leg – can’t remember if it was on my boxers or my leg but automatically thinking it was mine I removed it with my finger and put my finger in my mouth and swallowed to get rid of it. I quickly realised that this semen is unlikely to have been mine as the position of it on my leg didn’t make any sense. This escort had another client immediately before me and I was quick with her so between me and the previous client was no more than two minutes. My concern is that my leg or shorts (can’t remember now if it was directly on my leg or my shorts) had come into contact with this other clients semen which may have been on the bed (where I was of it was on my leg) or floor (where my shorts were) which I had then swallowed.

1. Given there had been no more than two minutes between me and the previous client, am I at at any meaningfil risk of ANY std from swallowing this semen (i'm concerned of passing something to my wife via cunnilingus). 

2. If yes can you also confirm my risk of Ureaplasma Urealyctum. I have read on this site that M. Gen and UU don't infect the pharynges but the following studies would seem to suggest otherwise (just for UU): https://www.ncbi.nlm.nih.gov/pubmed/115097 and https://www.ncbi.nlm.nih.gov/pubmed/20012733. If somehow i had got UU in my mouth from this incident would it be extremely difficult (in the same way as ghonnoreah) to pass to my wife via cunnilingus anyway? (I perform cunnilingus on her every time we have sex which is every day).

Welcome to forum.  I will be glad to comment.  The chain of events that you describe is most unlikely to have occurred.  Even if it had the event would not put you at risk for any STI.  Mutual masturbation is safe sex and even when partners get each others' genital secretions on each other or transfer them to each other, there is no risk for infection.  These pathogens are virtually immediately made non-infectious upon contact with the air/environment. 

I should add that while ureaplasma is NOT a pathogen.  The nearly 40 year old article you have found is out of date.  Ureaplasmas are a natural part of the bacterial present in the genital tract and, after extensive study, have been shown not to be pathogens.  I would not worry.  There is no reason for concern and no reason for testing of any sort.

Thanks for this Doctor, that's put my mind at ease for sure. 

Just to confirm a couple of things:

1. You mention mutual masterbation not being a risk. However, please confirm you understand that my concern is purely related to swallowing what potentially was another mans semen (the client who had seen the escort immediately before me). Please confirm that even if this semen I swallowed had been the client's immediately before me there would be no risk? (I have no concerns about me and the escort herself)

2. Secondly, you mention that pathogens are made non-infectious immediately after exposure to air.  Given I stated that my exposure was only two minutes (possibly even within two minutes) after this other client would have left semen there, can you confirm that this was still not a risk and immediately means immediately (ie. a few seconds)?

These are repetitive questions. Asking them repeatedly will not change the answer.

1.  Swallowing material transferred from your leg to your mouth on your finger is a no risk event.  I said it before, now I have said it twice.
2.  Immediately means immediately, just as no risk means no risk.
