[Question #2780] Endless Breakout HSV2 for 2 years now

At
first my breakouts would
come and go. I was taking small dosage of valacyclovir. It would help
for a few days then breakout would return. I had to steadily increase
dosage until now I am at half gram in morning and half at night. If I
only take 1 gram 1x per day it comes back. I have to spread out dosage
every 12 hours to prevent sores. That being said it never actually goes
away. I can see spot under skin (penis) where the virus is and would be
sores if not meds. I have actual sores that
change spots from under my arms to just above my buttocks. I am in very
good shape. My blood pressure is 129/70 and resting heart rate is 60bpm.
I eat very healthy and have developed an obsession with tracking my
food to make sure (I use myfitnesspal app) I get everything body needs. I
have been to doctors about it and they don't know what else to do
beside acyclovir. The issue for me is I read about this all the time and
cannot find anyone else who is talking about a *neverending* breakout.
I understand if my immune system is battling something that
takes away from its ability to fight HSV2. My doctor ran tests. All
tests came back saying I was very healthy(Chemistry Panel and Complete Blood Count, Hemoglobin A1C,
Thyroid Stimulating Hormone (TSH), Full STD Screening). I have tried
alkoline diet, epsom salt baths, Apple Cider Vinegar, Garlic... I only
drink water and eat fresh foods. My water is always 8ph. I havent drank
alcahol
in weeks. I gave it up for over a month before and it didnt help. I
drink water with fresh squeezed lemon daily. I meditate and workout. I
eat tons of spinach/kale/carrots/bell peppers/lentils/navy beans. My
stomach is 24/7 rumbling. As we speak right now it sounds like I havent
eaten in weeks. I eat very healthy 3 meals per day. Pea protein shake in
morning with almond milk, almond butter, kale, banana, kiwi, hemp seed.
Lunch is salad, with lentils and navy beans. What could be keeping my body from fighting off breakouts?

My follow up quesstion would be... What sort of tests could I have my doctor run to find an underlying problem that is keeping my body from fighting off breakouts? Do you think it could be valtrax? Do I need to let the virus run its course in order to have a break between breakouts? I read that it is very unlikely I can transmit oral herpes (I have it I can feel it in mouth). Does that low likely hood only apply when I can no longer feel it in my mouth? So if kissing partner since it is so rare to have hsv2 orally that chances are they wont even get it if kissing during breakout? I have looked over auto immune problems and it seems that I do not have any of them. I havent read anything online about another person being broke out for 2 years straight with no break in between. Have you heard of this before?

If you were my in-person patient,the first thing that I would do is swab the things that you are describing as a never ending outbreak. And to get the most accurate results, you should be off of medicine for 5 days. No one breaks out for 2 years straight but many people THINK they are having more outbreaks than they actually are. Also, HSV 2 genital herpes does not break out under the arms and it is also unlikely but not impossible that you have this in your mouth, even though you say you can feel it there - what does that mean, actually? If you had a real outbreak orally, you could transmit it to a partner, yes, by kissing.
The swab test that you need is PCR or NAAT (they are the same - just different ways of putting it).
It sounds to me like you are not only careful about your health but perhaps a bit obsessive.
It also doesn't make sense to me that you can see a spot under your skin where the virus lives. That's just not how this works. The virus lives at the base of the spine.
I would strongly suggest that you get these areas swab tested after being off meds for 5 days.

Ok I am a little scared to stop taking medicine for 5 days because my lips will start cracking and the sores will become visible/painful. My tongue currently feels like there are sores under the skin. Almost as though there are little splints in it. I can feel a spot inside of my lip that I have grown to expect is the virus. When I touch it with my tongue it feels like a small tapioca. When I pull my lip out and inspect the inside of mouth there are little bumps everywhere. All of the pain is on my tongue while on medication. The sore recently moved from the tip of my tongue to the back of my tongue on both sides. Almost feels like I bit it really hard and in "healing" stage. Obviously thats not the case. The top of my tongue feels like I burned it with hot food. Also not the case. The sores I described under my arm were even spacing between arm pits and hip. I had one on one side when it went away another one came out on other side. Felt like a bug bite but it always comes back at that exact location. The part on my penis where I said I can see it under skin is on the head. It may not be "under" skin but the color of the circle is a lighter color almost as if it is a sunspot that finally healed and is beginning to tan. That being said it doesnt ever heal and when I stop taking medicine that is are of sores. I go to VA. Should I have them set meeting with infectious disese and stop taking meds 5 days before I see them? What do you think and what should I do?

The thing is, if swabs are done while you are taking medicine, they will not be accurate. Herpes does not cause cracking lips. It also very rarely impacts the tongue. It also rarely impacts the head of the penis, far more often the shaft of the penis.
My personal opinion here is that you are way overestimating how frequently the virus is active in your body and where

I had a swab completed on my penis and I also had blood test done. Swab was on a sore that was on shaft of penis. The blood tests were done after. I also had traces of rocky mountain spotted fever however my body built antibodies and I was told that was no more a concern. They put me on doxycycline and that seemed to make it go away over first few days. My stools were getting back to normal but by day 3 the virus seemed to have fought back. I continued to take for next 2 weeks with no change. Doctors said if there was RMSV left it was enough doxy to stop it completely.
I should also mention that I looked up the type of cracking I was talking about on my lips and there are other pictures that look like what I have on google images. I forgot to mention before it also severely impacts my throat. I cannot emphasize enough the pain in swallowing when I am off meds. I have not pooped normal consistently in 2 years. It is hard for me to believe that I am overestimating the amount of time virus is active in my body because I see and feel the sores. If this isnt hsv2 then what else could it be? Certainly it isn't my imagination. There is a deep red spot on hole of penis that looks exactly like rash from herpes. My scrotum also has burning tingle and light redness like a rash from hsv2. Please give me some ideas on what it could be if not HSV2. I will certainly have to take off work in order to stop taking meds for 5 days.

I just confirmed the pain I am feeling on tounge is little clusters of red dots that are consistent with other pictures of HSV2 on tongue. Why would they not ever go away? You mentioned that sometimes people think they are having an outbreak but really arent. Can you help me understand how I could be making that mistake if there are always visual signs of sores? I am having a very hard time understanding that.

You didn't tell me exactly how you were diagnosed with HSV 2 in the first place - blood antibody test, swab test?

Have you or have you not had a laboratory diagnosis of HSV 2?

Yes. My private doctor is the one who completed the swab. He sent it off for review and called to confirm I was hsv2 positive. My VA doctor completed the blood test and said I was positive. Then I took another and he said I was negative. I challenged him that that couldnt be right so I took another one and when that one came back he said I was positive.

OK, good to know. I don't have other answers for you except to suggest that you get these areas that you are certain are outbreaks to confirm that they actually are.
